Job Description
Patient Navigator Duties Initiating contact with and responding to requests from patients, other team members and associated healthcare agencies and providers Assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way. Provide patients, relatives and other services with correct information Operate clinical computer systems and maintain high quality data entry. Communicate with other agencies like transport, phlebotomy, hospital etc. Maintenance of Records Accurately record details Make sure patient information is entered accurately and stored confidentially.
Record on the SystmOne system on daily basis outgoing prescriptions. Carry out range of clerical duties, as required
