Job Description

The postholder will:o Pay particular focus to the tracking of patient overdue follow-upappointments, introducing robust systems to avoid 'lost to follow up'.o Validate and monitor the Trust's performance against the Referralto Treatment 18 week waiting times standards, ensuring patientsreceive treatment within the required timescales through monitoringand auditing elective patient pathways.o Work as a member of the pathway tracking clinical team acting asa 'local expert' on patient pathway RTT 18 week guidance,reporting mechanisms and service improvements that will improvepatient experience & facilitate the provision of accessible and highquality care.o Be key in overseeing RTT 18 week performance managementacross the specialties, providing expertise & support for thedelivery & achievement of patient RTT standards.
Main duties of the job
o To actively monitor the departments overdue follow up lists, prioritising patients appointment dates and working towards clearing the waiting listso To work closely with the General Manager, Assistant Service Managers, , Clinical Office Team Leaders, Appointments Centre, Clinical Office Staff and Outpatient teams, Clinical staff and nursing staff within the division to ensure patients are actively managed through patient pathways.o To ensure that appropriate requests for investigations are monitored following the clinician's instructions. If diagnostic delays occur or appear imminent liaise with the relevant service for action.o To monitor patient's pathways from referral, through diagnostics, treatment planning and treatment, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process and report to clinical offices any discrepancies.o To ensure all information relating to the patient's pathway clearly identifies all relevant target dates to aid other users of the relevant systems, such as Cerner in allocating resources appropriately.o To provide, local training and support to other staff within the Trust regarding Referral to Treatment rules, processes and procedures.o To adhere to the Trust's Joint Access Policy and related national RTT Frameworks.
