A great opportunity has now arisen for a Patient Safety Midwife at Queen Elizabeth Hospital, Woolwich who has significant band 6 experience or is a current band 7?



If you interested in patient safety, risk management and promoting a positive culture of learning then this is the post for you. This role will enable to you to develop professionally by gaining experience and insight into risk management policies and processes and learning how to evaluate and grade clinical incidents, ensure incidents are reviewed in a timely manner and participate in management activities.



You will be responsible for reviewing clinical and non-clinical incidents within maternity which have been submitted via the Trust incident reporting system; appraising the incidents, identifying and investigating issues, themes or trends that may arise and developing action plans if required. Other aspects of the role include compiling reports for Directorate meetings, supporting the Clinical Governance Lead in maintaining action logs and the risk register, representing the maternity patient safety team as required and partaking in audit and guideline development.



The role requires a reasonable amount of previous clinical experience, IT skills, ability to communicate to members of the team and basic report writing skills



You will be required to participate within and co-ordinate the risk management (clinical and non-clinical) processes for the maternity services.



You will be required to assist and stand in for the clinical governance manager if required.



You will be responsible for participating in co-ordinating and investigating reported incidents within maternity services/ delegating and overseeing the investigation where appropriate.



You will be required to provide feedback to staff by way of quarterly, annual and ad-hoc risk reports and briefings as well as preparing documentation and evidence for relevant risk and patient safety meetings.



You will monitor and report the progress of implementation of action plans arising from incidents, complaints and claims and identify gaps in practice related to the recommendations from clinical risk incidents and serious untoward incidents.



You will also participate in the Complaints process and support the midwifery, medical, nursing, and any other staff with timely responses to complaints within the LGT Complaints Procedure if necessary.



The successful applicant will also be involved in the implementation of the PSIRF strategy as per the plan laid out by the Trust. A familiarity with the PSIRF principles would therefore be useful.

Post involves but not limited to:



The post holder will, as part of the Risk team, co-ordinate risk management within Maternity. He/she will work with the Clinical Governance Manager to ensure there is a proactive risk management infrastructure in place for the Maternity services, and to develop clear action plans in response to incident investigations. The Post holder will work with midwifery matrons, Clinical Practice Facilitators and Practice Development Midwives (Education Team) to improve standards of care.
