Job Description

KEY DUTIES AND RESPONSIBILITIES Patient Care Assesses, plans and evaluates care to a complex and dynamic caseload of patients, demonstrating analytical thinking, decision making, professionalism and leadership to ensure a high quality patient experience. Adopts an integrated care approach to meeting an individual patients needs across services through collaboration within a multi-disciplinary team setting, including the patients families and carers in decision making. Works within the Mental Capacity Act to advocate for the patient's best interests. Applies expert knowledge in palliative care to symptom control, recognition of dying and advance care planning.



Strategy and Service Development Actively contributes to appropriate service developments and quality improvements. Training, Education and Staff Development in accordance with local and national policies and drivers. Undertakes annual mandatory training updates and other relevant courses in line with organisational and local policies. Engages in the appraisal process and acts as appraiser of others as appropriate.



Provides mentorship and/or clinical supervision to staff and learners to aid the application of theory to practice. Shapes services for care home residents and house bound patients in accordance with local and national policies and drivers. Monitors and challenges risk in relation to their own practice and that of colleagues whilst assuming responsibility for own practice. Information Resources/Information Systems Undertakes critical analysis of contemporary research studies and incorporates valid research findings into practice.



Identifies and evaluates areas which require further research or audit and instigates collaborative research/audit projects. Autonomy/Scope within Role Practices competently and autonomously without direct supervision in accordance with professional code of practice and local policies and procedures. Collaborates with other relevant professionals to develop clinical protocols and integrated care pathways within area of expertise or speciality to benefit the delivery of clinical services. Monitors and leads improvements to standards of clinical care through supervision of practice, clinical audit, evidence-based practice, teaching and supporting professional colleagues and the provision of skilled professional leadership.
