About the Practice/Department/Team



PCNs (Primary Care Networks) were introduced in July 2020, as part of the NHS Long Term Plan, to encourage collaborative working across GP surgeries within local communities, providing additional services and programmes to support population health. Since inception, the scope and role of a Primary Care Network is ever evolving, with new initiatives and services being introduced frequently.

St Neots PCN is an average sized yet forward-thinking Primary Care Network (PCN) comprising of three practices within central St Neots and Great Staughton. We have built and developed a growing pharmacy team and recognise the value that these roles will bring to our practices and our patients, we now look forward to growing our PCN team. You will be joining an established, dynamic and varied team, whose aim is to provide exemplary patient care; finding innovative solutions in general practice to deliver the best care we can to our patients

Job Introduction



We have an exciting opportunity for an Advanced Nurse Practitioner to join our team within St Neots PCN, where you can make a positive contribution to patient care and peer support and supervision.

This role provides the opportunity to work across PCNs sites and to become involved in Population Health management across this PCN.

As an Advanced Nurse Practitioner you will be acting within your professional boundaries, providing care for patients from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care with the support of the PCN Clinical Lead as required. You will demonstrate safe, clinical decision-making and expert care for patients within the Primary Care Network (PCN), whilst working autonomously and taking full accountability for patient encounters. You will work within your professional boundaries as part of the Multi-Disciplinary team (MDT) across the PCN, supporting the delivery of the Primary Care Directed Enhanced Service to meet the needs of patients.

The Advance Nurse Practitioner will undertake duties within the Practice itself thus ensuring contemporary clinical skills and credibility as well as ongoing integration with the wider team and services. This would include review and care of patients attending the Practice (for pre-planned services or same day urgent care) and include diagnosing and treating patients presenting with minor illness, seeing and advising people in respect of their continuing medical and nursing needs.

This exciting opportunity allows the Advanced Nurse Practitioner to retain their generalist knowledge and will keep their role fresh and invigorating, whilst supporting work in all four pillars of advanced practice.

What you will do



Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team. Manage undifferentiated undiagnosed condition and identify red flags and underlying serious pathology and take appropriate action Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice. Actively take a personalised care approach and population centred care approach to enable shared decision making with the presenting person. Complete the relevant training in order to provide multi-professional clinical practice and CPD supervision to other roles within primary care, for example first contact practitioners and the personalised care roles.

Essential Criteria



RGN Qualification Either graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centre's ePortfolio (supported) Route; NMC 1st level Registration Educated to master's Level - Nurse Practitioner Pathway Is working at a master's level aware or equivalent that encompasses the four pillars of clinical practice, leadership and management, education, and research, with demonstration of core capabilities and area specific clinical competencies. Independent Prescribing Qualification Evidence of continued professional development.
