The post holder is a qualified pharmacist working within their professional boundaries to support and collaborate with a team of pharmacists in a General Practice setting. They will receive ongoing development, management, and mentorship from the Medicines Management Lead. As part of a multi-disciplinary, patient-facing team, the pharmacist will take the lead in managing minor ailments and contributing to chronic disease management. They will also conduct clinical medication reviews, with a focus on proactively supporting patients with complex polypharmacy needs.
The role includes providing key support to general practice staff in handling prescription and medication-related queries. Responsibilities also involve supporting the repeat prescription system, processing acute prescription requests, performing medicines reconciliation following transitions of care, and promoting safer prescribing practices. The pharmacist will offer expert clinical advice while considering both the healthcare and social care needs of patients across the practice(s). In addition, the post holder will offer clinical leadership in medicines optimisation and quality improvement initiatives.
They will also be involved in managing specific elements of the Quality and Outcomes Framework (QOF) and enhanced services. A crucial aspect of the role is fostering collaboration between the practice and community and hospital pharmacy teams to optimise skill mix, enhance patient outcomes, increase access to care, and support workload management. This role is central to delivering high-quality care and improving operational efficiency, and therefore demands a motivated, proactive professional with a passion for excellence in primary care.
