As part of our multidisciplinary team, you will deliver the following in accordance with NHS England's Additional Roles Reimbursement Scheme:
Patient Care & Assessment:
Clinically assess and treat patients using expert knowledge of medicines for specific disease areas.
Prescribing:
Ideally an independent prescriber, or willing to undertake the qualification (funded and supported by the PCN).
Chronic Disease Management:
Manage care of patients with long-term conditions and undertake medication reviews, particularly for complex cases including older adults and those in care homes.
Medicines Optimisation:
Provide specialist input in medicines use, addressing public health needs and tackling health inequalities.
Quality Leadership:
Contribute to QOF and enhanced services, including antimicrobial stewardship and quality improvement projects.
Structured Medication Reviews:
Support patients to optimise therapy, reduce waste, and encourage self-care.
Integration & Collaboration:
Work within the PCN Clinical pharmacy team to provide excellent care for practice patients.
Professional Networks:
Build relationships with pharmacy professionals and contribute to shared care protocols and research activity.
Essential Requirements
Registered Pharmacist with the
General Pharmaceutical Council (GPhC)
Completion of the CPPE Primary Care Pharmacy Pathway
(mandatory under the ARRS scheme)
Clinical experience in a healthcare setting
Strong communication and interpersonal skills
Ability to work independently and within a multidisciplinary team
Commitment to ongoing professional development and clinical supervision
Computer literacy and familiarity with clinical systems
Desirable Requirements
Experience working in general practice or primary care
Independent prescribing qualification (preferred)
Experience with medication reviews and chronic disease management
Knowledge of local prescribing guidelines and formularies
