Brackley and Towcester PCN Ltd is seeking an experienced Clinical Pharmacist to join our dynamic healthcare team at Towcester Medical Centre. This is an exciting opportunity to make a real difference in primary care, working closely with GPs, nurses, and other healthcare professionals to optimize patient medication management and outcomes.
Key Responsibilities
As part of our multidisciplinary team, you will deliver the following key responsibilities in accordance with NHS England's Additional Roles Reimbursement Scheme requirements:
Patient Care & Assessment:
Work in a patient-facing role to clinically assess and treat patients using expert knowledge of medicines for specific disease areas
Prescribing:
Not essential but preferred.
Chronic Disease Management:
Manage care of patients with chronic diseases and undertake clinical medication reviews, particularly for complex polypharmacy cases including older people, care home residents, those with multiple co-morbidities (frailty, COPD, asthma), and people with learning disabilities or autism (STOMP programme)
Medicines Optimisation:
Provide specialist expertise in medicines use while addressing public health and social care needs, helping to tackle health inequalities
Quality Leadership:
Lead person-centred medicines optimisation, quality improvement initiatives, and contribute to QOF and enhanced services, including antimicrobial stewardship
Structured Medication Reviews:
Support patients through structured reviews to optimize medication effectiveness, reduce waste, and promote self-care
Integration & Collaboration:
Lead integration efforts with wider healthcare teams (community and hospital pharmacy) to improve patient outcomes and manage practice workload
Professional Networks:
Develop relationships with pharmacy professionals across PCNs and the wider health and social care system
Shared Care & Research:
Take central role in shared care protocols, clinical research with medicines, and liaison with specialist and community pharmacists
Clinical Supervision:
Participate in professional clinical networks with access to appropriate clinical supervision and peer support
Essential Requirements
Registered Pharmacist with the General Pharmaceutical Council (GPhC)
Must have completed an approved 18-month training pathway (such as CPPE Clinical Pharmacist pathways) that equips you to practice safely in primary care
CPPE Primary Care Pharmacy Pathway completion is mandatory
(requirement under Additional Roles Reimbursement Scheme)
Clinical experience in a healthcare setting
Strong communication and interpersonal skills
Ability to work independently and as part of a multidisciplinary team
Commitment to continuing professional development and clinical supervision
Computer literacy and familiarity with clinical systems
Desirable Requirements
Previous experience working in general practice or primary care
Experience with clinical medication reviews and chronic disease management
Knowledge of local prescribing guidelines, formularies, and antimicrobial stewardship
Independent prescribing qualification
