About the Role



We are looking for a compassionate and experienced Practice Nurse with a special interest in Frailty, ideally qualified as an Advanced Nurse Practitioner (ANP), to join our innovative Primary Care Network (PCN) under the ARRS scheme.

This role will be based across two GP practices in Hertford, supporting the delivery of high-quality, proactive care to our older and more vulnerable patients. You will play a key role in advanced care planning, completing and reviewing ReSPECT forms, and leading our frailty multidisciplinary team (MDT) meetings.

Key Responsibilities



Lead on frailty identification, assessment, and management across both practices. Conduct comprehensive geriatric assessments and develop personalised care plans. Facilitate and support Advanced Care Planning discussions with patients and families. Complete and maintain ReSPECT forms in collaboration with the wider clinical team. Take a lead role in organising and chairing frailty MDT meetings, ensuring coordinated, patient-centred care. Work collaboratively with GPs, pharmacists, care coordinators, social prescribers, and community teams. Support care homes and housebound patients with holistic, preventative care. Contribute to reducing unplanned hospital admissions and improving quality of life for frail patients. Provide mentorship and clinical leadership where appropriate.

What We're Looking For



? Registered Nurse with current NMC registration
? Experience in primary care and managing patients with complex needs
? Strong interest or qualification in frailty care
? ANP qualification (desirable but not essential)
? Independent prescribing qualification (desirable)
? Experience with ReSPECT forms and advanced care planning (preferred)
? Excellent communication, leadership, and team-working skills
? Commitment to continuous professional development

Pay: 47,809.00-67,665.00 per year

Expected hours: 20 - 37.5 per week
