We are made up of four GP Practices which are based in Congleton and Holmes Chapel; Holmes Chapel Health Centre, Lawton House Surgery, Meadowside Medical Centre, and Readesmoor Medical Centre. We, together as a PCN have a patient list size of approximately 50,000, for whom we provide General Practice services. By working together with our local providers of care we are improving the patient's experience of primary care. We are in the Cheshire East Council geographic area.

Our vision at Congleton and Holmes Chapel (CHOC) Primary Care Network (PCN), is to support the NHS long term and plan protecting the long term- sustainability of the general practice model. We aim to bring together collaborative working for health, social care and the voluntary sector improving systems and processes enabling increased time to deliver improved patient care. We will achieve more by harnessing the strengths of our stakeholders. Our mission is to place the patient at the heart of everything we do within the community delivered by a multi-disciplinary team to improve our patient's health.

Vernova Healthcare CIC is the employing body for CHOC PCN Additional Role staff. Vernova is owned and supported by all of the GP practices in Eastern Cheshire.

The successful candidate will join a growing PCN Team, (including Clinical Pharmacists, FCPs, PA, SPLWs, ANP, Paramedic Practitioners and MHPs) and will be supported by the PCN Operational Manager and Digital and Transformation Lead. Regular development sessions, supervision, teaching, training, and opportunities to improve skills will be provided, linking in with the PCN Clinical Educational Lead as and when appropriate.

The Role



CHOC PCN are looking for an enthusiastic and motivated GP Assistant who shares our values and is committed to providing excellent care, to join our growing team.

As a GP Assistant you will support our GP practices and work closely within the PCN Same Day Access Service, which provides on the day acute care to the patient population. You will be working closely with our ANP / ACP and paramedic practitioner team. The successful candidate will share our values and have the energy, skills and enthusiasm to contribute to shaping how we support our patients. You will be required to act within your professional boundaries, supporting and working alongside a multi-disciplinary team within general practice and a Primary Care Network.

To meet the requirements of the Same Day Access Service, the post holder will be required to work Monday to Friday from 8am in the morning.

This role is part of the PCN Additional Roles Reimbursement Scheme.

The GP Assistant will:

Support the smooth running of clinics by performing more routine administration and clinical tasks on behalf of the clinical team.

Assisting and supporting with the management of the day to day operations of the SDAS, including patient queries, rotas and support of administrative tasks and responsibilities.

Developing, amending and updating appointment templates for the SDAS and other PCN services.

Arranging appointments, referrals, test and follow up appointments of patients.

Completing simple clinical observations / investigations as directed and as appropriate to skill set e.g. dipstick urine, taking blood pressure, phlebotomy.

Helping the SDAS team liaise with outside agencies e.g. getting an on call doctor or liaising with secondary care for emergency admission, liaising with the ambulance service.

Supporting the clinical team with immunisations e.g. flu and covid campaigns

Supporting the clinical team with MDT meetings with a focus around palliative care / complex families / mental health / drug & alcohol MDTs.

Supporting practice teams with QOF reviews / patient recalls.

Support the PCN team with the collation of patient and staff feedback, including records of SEAs / compliments / complaints.

Support the PCN Clinical Education Lead with the placement of students across the PCN practices.

Track and record evidence of your experience against the national competency framework for GPAs and undertake any additional training required as part of the role, including participation in an annual individual performance review / appraisal.

Be aware of your own professional boundaries and what to do when.

Support GP Practices with administration tasks on an ad hoc basis.

Person Specification:



Essential qualifications, experience and skills:



A minimum of five GCSEs, ideally at grades A -- C, but equivalent level of work experience will be considered.

NVQ/Apprenticeship level 2 Business & Administration (or relevant equivalent).

Excellent IT skills

Experience of using software packages such as EMIS, EMIS Enterprise, Docman, AccuRx, Microsoft office.

Experience of working in Primary Care.

Good communication and including good telephone and inter-personal skills.

Organised, good attention to detail.

Excellent customer care focus.

Ability to handle demands with professionalism, tact and diplomacy.

Work as part of a team to support the overall delivery of the team/department.

Ability to maintain confidentiality.

Ability to work with a methodical and careful approach and to remain calm under pressure.
