50,000-60,000-dependent on experience and qualifications. This role is also open to Advanced Clinical Pharmacists that fully meet ARRS scheme criteria .

About Us



Mosaic Primary Care Network consists of 3 practices (Crown Heights Medical Centre, Odiham & Old Basing Health Centres and Brambly's Grange Medical Practice) located in or near Basingstoke. We have a combined patient population of circa 60,000 patients. We are looking for an experienced lead pharmacist to join our team.

The PCN is a registered learning environment, so all our ARRS staff are supported by an EELS lead to ensure we meet the aspirations of both the team member and the surgeries.

Essential Information



You will need to work across the PCN practices.

You must be a registered pharmacist with experience in Primary Care.

You must be a prescriber or currently completing training to prescribe.

You should have some experience in leading a team.

You will need to hold a full driving licence and have access to your own transport.

Option of one day working from home, once established.

Job Overview



To improve patients health outcomes and the efficiency of the primary care network (PCN) pharmacy team by providing direct, accessible and timely medicines expertise as part of a multi-disciplinary team. To pro-actively complete workloads relating to medicines optimisation issues from other clinical staff to improve patient care, safety and outcomes.

The Lead Clinical Pharmacist must be a prescriber and registered with the General Pharmaceutical Council (GPhC). This role requires enrollment in (if not already qualified from) an approved 18-month training pathway or equivalent that equips the Clinical Pharmacist to be able to practice and prescribe safely and effectively in a primary care setting, for example, the CPPE Clinical Pharmacist training pathways.

The summary of the role includes:

a. work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas;

b. be a prescriber, or completing training to become a prescriber and work with and alongside the general practice teams;

c. be responsible for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially older people, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism (through STOMP - Stop Over Medication Programme)

d. provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCN's practices and to help in tackling inequalities;

e. provide leadership on person-centered medicines optimisation (including ensuring prescribers in the practices conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services;

f. through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote selfcare;

g. have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload;

h. develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system;

i. take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning disabilities), liaison with community pharmacists and anticoagulation; and

j. be part of a professional clinical network and have access to appropriate clinical supervision. Each clinical pharmacist must receive a minimum of one supervision session per month by a senior clinical / advanced practice pharmacist;?

k. the senior clinical / advanced practice pharmacist must receive a minimum of one supervision session every three months by a GP clinical supervisor;?

l. each clinical pharmacist/senior clinical pharmacist/advanced practice pharmacist will have access to an assigned GP clinical supervisor for support and development;

m. a ratio of one senior clinical / advanced practice pharmacist to no more than five junior clinical pharmacists, with appropriate peer support and supervision in place.?

k. undertake clinical audits as agreed with the prescribing lead;

l.receive and resolve medicine queries from patients over the telephone and/or face to face,

m. lead and coordinate the PCN pharmacy team and be a point of access for any support they may require such as meetings, educational needs, professional discussions,

n. be responsible for workflow optimisation and delivery of daily services,

o. coordinate & deliver appraisals for the pharmacy team,

p. work alongside GP practice staff as an integrated part of the practice teams.

Responsibilities underpinning the role



To develop and facilitate a good working relationship with all local providers of healthcare.

To work closely with the Prescribing Leads and Practice Managers to agree job objectives and timetable

To record personally generated information and maintain a database of information relating to the work done in the practice(s). Personally generated information includes information and records relating to audit and clinical work undertaken by the post holder, reference notes relating to clinical/technical information.

Maintain records of all work done in the practice for which the post holder is accountable through the correct use of the clinical system.

To maintain registration as a pharmacist and comply with appropriate professional codes.

As appropriate to the post, to maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, local and national service specifications, legislation and policy developments,

To agree objectives and a personal development plan and participate in the appraisal process.

To attend local, regional and national meetings of relevance as required by the practice.

To undertake any other duties commensurate with the post holder's grade as agreed with the post holder's line manager.

Role Delivery Specifics



On the day telephone and patient

facing medicines support



Provide support to the Duty Doctor Team by responding by telephone for patients with questions, queries and concerns about their medicines. Refer to Community Pharmacists for medicines use reviews (MURs) as necessary. Hold face-to?face appointments for patients requiring a clinical review.

Medicine information to practice staff and patients



Answer all medicine?related enquiries from GPs, other practice staff and patients with queries about medicines.

Out of stocks



Identify alternatives where GPs and other practice staff have been advised a Community Pharmacy cannot supply prescribed items.

Care home repeat prescribing



To be responsible for the authorisation of nominated Care Homes repeat prescriptions, and to pass to nominated GP upon completion.

Appliances and lymphoedema stockings



To support the GPs by reviewing appliance and stocking requests where they consider this is best dealt with by the post holder.

Management of medicines at discharge from hospital.



To reconcile medicines following transfer of care (discharge from hospitals, intermediate care and into care homes), including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they should have post discharge.

Set up and manage systems to ensure continuity of medicines supply to high?risk groups of patients (e.g. those with medicine compliance aids or those in care homes).

Medication review



Undertake medication reviews as agreed with the Prescribing Leads and produce recommendations for the GPs on prescribing and monitoring. Hold face-to?face appointments for patients requiring a clinical review

Care home medication reviews



Provide support to the GPs and Community Matron, as required, by undertaking clinical medication reviews with patients and produce recommendations for the GP on prescribing and monitoring. Work with care home staff and community pharmacies to improve safety of medicines ordering and administration to improve safety, reduce unplanned admissions and reduce waste.

Long-term condition reviews



See patients with single medical problems where medicine

optimisation is required (e.g., COPD, asthma). Ensure the review is completed in line with the practice template. Make recommendations to GPs and LTC Nurses for medicine improvements.

Cost saving programmes



Undertake changes to medicines (switches), as agreed with the Prescribing Leads, designed to save on medicine costs where a medicine or product with lower acquisition cost is now available without impacting on patient safety or health outcomes.

Information management



Analyse, interpret and present medicines data to highlight issues and risks to support decision making.

Medicines quality improvement



Undertake simple audits of prescribing in areas directed by the GPs, feedback the results and implement changes in conjunction with the practice team.

Training



Provide education and training to primary healthcare team on therapeutics and medicines optimisation.

Implementation of local and national guidelines and formulary recommendations



Monitor practice prescribing and make recommendations to GPs for medicines that should be prescribed by hospital doctors or subject to shared care .

Medicines safety



Implement changes to medicines that result from MHRA alerts, product withdrawal and other local and national guidance.

Support the practice in adopting local and national medication safety strategies.

Improve patient and carer understanding



Improve patient and carer understanding of confidence in and compliance with their medicines

Safety and quality of prescribing



Improve the safety and quality of prescribing following hospital admissions and attendance.

Care Quality Commission



Work with the practice managers and GPs to ensure the practice is compliant with CQC standards where medicines are involved. Undertake risk assessment and management and ensure compliance with medicines legislation.

Team Leadership



Manage a team of pharmacists and pharmacy technicians , ensuring that adequate time is allocated to appointments, referrals, tasks and documentation. Ensure adequate cover during periods of annual leave. Be a source of support for clinical queries. Complete appraisals and provide training and education for members of the pharmacy team. Communicate relevant information to all team members.
