To provide advanced clinical assessment and patient management within Primary Care, supporting the delivery of safe, effective, and high-quality healthcare services. The postholder will work within a defined scope of practice and will operate under the supervision and guidance of a Duty GP and other independent Clinicians. The role includes supporting long-term condition management, delivering minor illness clinics when required, undertaking home and care home visits, supporting clinical triage, and contributing to Quality and Outcomes Framework (QOF) activities.
Key Responsibilities
Conduct advanced clinical assessment and autonomous management of patients presenting with urgent or routine healthcare needs, within a defined scope of practice.
Undertake diagnostic investigations and deliver evidence-based treatment.
Support clinical triage processes to ensure appropriate prioritisation and patient flow.
Manage long-term and complex conditions, including participation in QOF-related activities to support quality care standards.
Conduct home visits and provide care home visits and ongoing support for patients in the community.
Deliver minor illness clinics as required to enhance patient access and service continuity (training can be provided).
Maintain accurate and contemporaneous clinical records in line with organisational policies and clinical governance standards.
Contribute to preventative health initiatives and population health programmes.
Offer clinical leadership and mentorship to junior colleagues or students.
Comply with all safeguarding, medicines management, and infection prevention protocols.
Actively participate in service development, audit, and quality improvement initiatives.
Work collaboratively as part of a multidisciplinary team.
Person Specification
Qualifications / Registration:
Registered Paramedic with the Health and Care Professions Council (HCPC).
Evidence of ongoing professional development.
Advanced clinical skills appropriate to Primary Care practice.
Experience:
Experience in urgent and primary care settings preferred.
Experience managing long-term and complex conditions preferred.
Experience delivering home and care home visits.
Experience in clinical triage and minor illness management.
Knowledge and Skills:
Strong clinical reasoning and decision-making skills.
Ability to work autonomously within a defined scope of practice.
Knowledge of primary care systems, QOF, and preventative health initiatives.
Excellent communication and interpersonal skills.
Ability to contribute to service improvement and audit processes.
