About Us
Mosaic Primary Care Network consists of 3 practices (Crown Heights Medical Centre, Odiham & Old Basing Health Centres and Brambly's Grange Medical Practice) located in or near Basingstoke. We have a combined patient population of circa 60,000 patients.
We are looking for an experienced paramedic to join us at Odiham and Old Basing Health Centres , ideally with a prescribing qualification.
Essential Information
You will need to work from both Odiham and Old Basing Health Centres
You will need to hold a full driving license and have access to your own transport
Days of work are Mondays, Wednesdays, Thursdays and Fridays.
Job summary:
To provide a high-quality, cost-effective service to the patients of the PCN; diagnosing and, if qualified, prescribing treatments and medication while working within defined procedures and protocols as laid down by the GPs.
You will be working as part of a clinical team with GPs, Practice Nurses and supporting Healthcare Assistants.
Your primary task will be to work within the duty team, assessing and treating same day acute presentations. These will typically be patients presenting with a wide range of minor illness, urgent and same day requests for treatment. There is scope for chronic disease management in addition to same day care, depending on experience. You will be providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.
You will need to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills. You will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.
You may be asked to do home visits depending on need but the practice has a low home visit rate and is supported by a home visiting service, so the need will be minimal. You would not be asked to do palliative or end of life visits. Individual practices may wish to invest in training to enable the job holder to support the team in chronic disease management.
Job Responsibilities:
Clinical Practice
You will act as an autonomous clinician with the assistance of the clinical team
Work as part of an MDT within the PCN
Assess and triage patients, including same day triage and as appropriate provide definitive treatment (including prescribing medications following policy, PGD, NICE and local clinical guidelines and local care pathways, or make necessary referrals to other members of the primary care team.
Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to the PCNs Social Prescribing service, and where appropriate, other community or voluntary services
Be able to perform and interpret ECGs, investigatory procedures as required and undertake collection of pathological specimens including blood sample, swabs and other samples within the scope of practice
Support the delivery of anticipatory care plans and lead certain community services
Provide an alternative model to urgent and same day GP home visit for the network and clinical audits
Communicate at all levels across organisations ensuring that an effective, person-centred service is delivered.
Communicate pro-actively and effectively with all colleagues across an MDT
Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings and treatment choices.
Ensure clinical practice is safe and effective, and remains within boundaries of competence
Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
Take a comprehensive patient history, carry out physical examinations and use expert knowledge and clinical judgement to identify the potential diagnosis
Refer patients for investigations and/or the appropriate specialist where appropriate
Make a final diagnosis
Independently decide on and carry out treatment, including the prescribing of medicines, if qualified
Use your practice experience to plan and provide skilled and competent care to meet patients' health and social care needs, involving other members of the wider health care team as appropriate
Ensure the provision of continuity of care including follow-up consultations / visits
Assess and evaluate, with patients, the effectiveness of the treatment and care provided and make changes as needed
Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care
Administration and professional responsibilities
Participate in the administrative and professional responsibilities of the practices within the PCN
Ensure accurate and legible notes of all consultations and treatments are recorded in the patient's computer record
Ensure accurate completion of all necessary documentation associated with patient care
Ensure collection and maintenance of statistical information required for regular and ad-hoc reports and audits
Attend and participate in clinical meetings
Delivery a quality service
Recognise and work within own competence and professional code of conduct
Prioritise, organise and manage own workload in a manner that maintains and promotes quality
Deliver care according to NSF, NICE guidelines and evidence-based care
Assess effectiveness of care delivery through self and peer review, benchmarking, audit and formal evaluation

Support and participate in shared learning across the practice
Understand and apply legal issues that support the identification of vulnerable and abused children and adults, and be aware of statutory requirements

Utilise and demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment

Training and personal development
Training requirements will be monitored by yearly appraisal and will be in accordance with PCN requirements
Personal development will be encouraged and supported by the PCN. It is the individual's responsibility to remain up to date with recent developments
Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development
Develop and maintain a Personal Learning Plan
Liaison
There is a need to work closely with all members of the PCN including reception and administrative staff, as well as the medical and nursing teams
There is also the need to establish and maintain good liaison with other agencies, including the Community Nursing Team, Social Care Services and Secondary Care
Confidentiality
In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health or other matters. They do so in confidence and have the right to expect that staff will respect their privacy and act appropriately
In the performance of the duties outlined in the job description, the post holder may have access to confidential information relating to patients and their carers, practice staff, and other healthcare workers. They may also have access to information relating to business organisation. All such information from any source is to be regarded as strictly confidential
Information relating to patients, carers, colleagues, other healthcare workers or the business of the PCN may only be divulged to authorised persons in accordance with PCN and practice policies and procedures relating to confidentiality and the protection of personal and sensitive data
Any breach of confidentiality may result in disciplinary proceedings

Qualifications and person specification
Essential
Educated to degree/diploma level in Paramedicine or equivalent experience.
Registered with the Health and Care Professions Council (HCPC).
Completed their two-year 'Consolidation of Learning' period as a "newly qualified paramedic".
Further three years' experience as a band 6 (or equivalent) paramedic
Working towards developing master's level or equivalent capability in paramedic areas of practice and, within six months of the commencement of reimbursement for that individual (or a longer time period as agreed with the commissioner), has completed and been signed off formally within the clinical pillar competencies of the paramedic FCP/AP roadmap to practice
