Job Description

Main Duties and Responsibilities Manage dedicated pharmacist-led appointment slots to reduce GP workload and locum use. Provide timely response to medication-related queries and urgent prescription requests. Conduct Structured Medication Reviews (SMRs) for frail, polypharmacy, or high-risk patients. Lead on diabetes and hypertension clinics, optimising medicines and lifestyle management.



Undertake targeted reviews of patients at risk of unplanned hospital admissions. Reconcile medicines following hospital discharge, outpatient appointments, or care transfers. Identify and rectify discrepancies or unexplained changes in medication. Ensure continuity of medicine supply for vulnerable patients, including care home residents.



Support safe administration, monitoring and ordering of medicines in care homes. Provide clinical triage and management of minor ailments within professional competence. Promote self-care, medicines adherence, and appropriate use of NHS services. Support implementation of MHRA alerts, NICE guidance, and local formulary changes.



Promote antimicrobial stewardship and contribute to prescribing safety and governance. Deliver QOF and PCN DES prescribing objectives. Provide medicines information and training to GPs, nurses, HCAs, and other staff. Participate in MDT reviews, case discussions, and virtual ward rounds.



Lead prescribing audits, clinical governance, and quality improvement initiatives. Contribute to local incentive schemes and public health campaigns. Mentor junior pharmacists, trainees, and pharmacy students across the PCN.
