Woodcote Medical is delighted to offer an exciting opportunity for a dedicated and proactive PCN Pharmacy Practitioner to join our friendly multi-disciplinary team. This is a fantastic role for a pharmacist who is passionate about high-quality patient care, medicines optimisation, and collaborative working within general practice.

As part of the Purley & Coulsdon PCN, you will work closely with our GPs, nurses, ANPs, paramedics, physiotherapists, and administrative teams to deliver safe, effective, and person-centred care across our population of approximately 18,000 patients.

We are a supportive, teaching practice and take pride in helping clinicians develop, thrive, and build long-term careers in general practice.

Main Duties of the Role



As a PCN Pharmacy Practitioner, you will be a valued member of the clinical team, responsible for:

Clinical Work



Conducting structured medication reviews, focusing on high-risk groups such as frail elderly or patients with polypharmacy Running minor ailment and long-term condition clinics, managing chronic diseases where appropriate Supporting medicines optimisation initiatives across the PCN Reviewing and acting on pathology results Managing and reconciling medications following hospital discharge and other care transitions Leading on repeat prescription processes, including reauthorisation and safe prescribing Providing clinical leadership on prescribing quality, safety and cost-effective practice

Operational & Collaborative Work



Acting as a key point of contact for medicines queries for clinicians, reception teams and patients Supporting PCN quality and outcomes framework (QOF) and IIF targets Contributing to audits, service improvement, and implementation of local and national guidelines Supporting digital tools and pathways to enhance prescribing and clinical workflow
What We're Looking For

Essential

Master's degree in Pharmacy and GPhC registration Completion of, or near completion of, CPPE Primary Care Pathway Strong clinical skills with awareness of common acute and long-term conditions Excellent communication skills and the ability to work collaboratively across MDTs A proactive, compassionate approach and a commitment to improving patient outcomes
Desirable

Independent Prescribing Qualification (or willingness to undertake with our support) Experience working in general practice or across primary care Experience in medicines optimisation or polypharmacy clinics
