30/5/25Key details/responsibilities:
Provide support and guidance to all participants that have been registered onto the project. To develop ongoing action plans with participants which will be reviewed on a regular basis. To provide one to one support and group activities to aid the development of a participant in their recovery. Work alongside the team structure in delivering services and outcomes for the scheme. To achieve project targets and outcomes set by Welsh Government. To support the team leader and referral agents in raising awareness of the service. To work in partnership with other practitioners to deliver effective interventions and support for people with Mental Health and/or substance use. Provide support and supervision to volunteers.
Job Types: Full-time, Fixed term contract
Contract length: 15 months
Pay: 24,347.00 per year
Schedule:
Day shift Monday to Friday Weekend availability
Work Location: On the road
