JOB TITLE: Support Worker
LOCATION: North West London
ACCOUNTABLE TO: Project/Deputy Manager
INTRODUCTION: Adolphus Care provides support and care to people with mild to moderate learning difficulties and/or mental health issues to assist them to live a full and active life. Service users require support from staff to assist in the completion of daily tasks around their home as well as accessing educational and employment opportunities in the community. We empower our service users therefore, we assist them to complete tasks themselves, NOT complete tasks for them.
Ideally we give our service users the life skills they need to move on to more independent accommodation in the community.
PRINCIPLE RESPONSIBILITIES:
SERVICE USERS:
To ensure:-
1. Service user's rights and responsibilities are upheld at all times.
2. Service users are supported in all aspects of social and domestic skills training.
3. Service users are fully involved with all aspects of their support planning and delivery.
4. Service users are supported in managing their finances and keeping appropriate records.
5. Appropriate leisure and other social activities are made available to service users in accordance with their needs and wishes.
6. Support and monitor service users use of prescribed medication and the keeping of appropriate records.
7. Each service user has a key/link worker allocated to them.
8. Appropriate communication with multi agency professionals, staff, relatives and advocates including the completion of reports etc where necessary.
9. Service users are supported to hold regular service user's meetings.
10. Service users are supported where necessary to engagements such as hospital appointments etc.
11. The confidentiality of service user's personal information.
12. Each service user is treated as an individual and their human rights are respected.
STAFF:
You are required to:-
1. Encourage the development and maintenance of good professional standards in practice and an appropriate attitude to the needs of the service users.
2. Undertake staff development and training.
3. Undertake regular individual staff supervision.
4. Engage in regular staff meetings - encouraging participation of all members of staff.
5. Ensure comprehensive records are meticulously maintained in respect of all service users, staff and the project.
6. Follow Action Plans and Person-Centred Plans monitoring and recording outcomes.
7. Ensure that families and multi-agencies are closely involved in the review of service user's progress.
8. Follow the Complaints and Guidance Procedure in accordance with Adolphus Care guidelines.
9. Maintain the welfare and safety of service users, staff, visitors and the Project in accordance with Health and Safety legislation.
10. Undertake shifts based on the published rota and around the needs of service users, subject to negotiation with the Project Manager. The post holder will normally need to available to undertake at least one sleepover per week.
FINANCIAL:
You are required to:
1. Maintain honesty at all times in your dealings with service user's funds and petty cash in accordance with the Adolphus Care Financial Management Guidelines.
2. Give best efforts to ensure all expenditure for the Project is kept within the budget limits set by the Project Manager.
3. Where it is necessary to exceed a spending limit you are required to inform the Project Manager before you do so.
4. Identify and notify the Project Manager of any significant changes in service user's support needs, which may have resource implications.
5. Be responsible for other budgets allocated to the Project such as Petty Cash.
GENERAL:
You are required to:
1. be fully conversant with the Company's Policies and Procedures.
2. Adhere to the Policies and Procedures of Adolphus Care.
3. Keep the Project Manager/Service Manager informed of specific areas of difficulty.
4. Undertake any other related duties commensurate within the general level of responsibility of the post as directed by the Project Manager/Service Manager.
