We are currently seeking a motivated and GPhC-registered Pharmacist to join our friendly and professional team, supporting medication management across all clinical departments.
Key Responsibilities:
Carry out medicines reconciliation and screening of drug charts on the ward.
Counsel patients on TTOs (To Take Outs) and medication changes to ensure safe discharge and full understanding.
Provide accurate and evidence-based medicines information, responding to clinical queries from healthcare professionals.
Aid and support the hospital's antimicrobial stewardship programme.
Carry out regular audits to maintain high standards of medication safety and compliance.
Collaborate closely with the wider clinical team to promote safe, effective, and personalised patient care.
What You'll Bring:
GPhC registration as a qualified pharmacist is essential.
Previous hospital experience is desirable but not essential - full training and development are available.
Excellent communication and organisational skills.
A proactive, patient-focused approach with a passion for clinical pharmacy.
