The Clinical Pharmacist will work as part of a multidisciplinary team in a GP practice . They will be responsible for supporting and improving medication safety, optimising medicines use, and providing expert clinical advice to patients and colleagues. This role aims to reduce GP workload and improve patient care by ensuring safe, effective, and evidence-based use of medicines.
Key Responsibilities:Patient-Facing Roles:
Conduct structured medication reviews (SMRs) for patients with long-term conditions, polypharmacy, or complex needs.
Support chronic disease management (e.g., hypertension, diabetes, asthma) by initiating and adjusting medications.
Provide advice and support to patients on the safe use of their medicines, side effects, and adherence.
Address medication queries from patients via telephone, video, or in-person consultations.
Clinical and Technical Duties:
Reconcile medications following hospital discharge or outpatient appointments.
Authorise repeat prescriptions and manage repeat prescribing systems.
Review prescribing patterns and ensure alignment with clinical guidelines and formularies.
Identify and address medication-related safety issues.
Support implementation of prescribing policies and national/local guidance.
Team Support and Education:
Work closely with GPs, nurses, and other clinicians to provide integrated care.
Educate and support practice staff on medicines management, including training for healthcare assistants and reception teams.
Collaborate with community pharmacies and other external partners to ensure continuity of care.
Auditing and Quality Improvement:
Participate in clinical audits and quality improvement projects.
Contribute to CQC inspections and ensure adherence to clinical governance standards.
Support QOF (Quality and Outcomes Framework) and IIF (Investment and Impact Fund) targets.
Person Specification:Essential:
GPhC-registered pharmacist.
Relevant postgraduate clinical qualification or willing to undertake (e.g., CPPE Primary Care Pharmacy Education Pathway).
Experience in patient-facing roles or primary care settings.
Strong communication, organisational, and IT skills.
Ability to work independently and as part of a team.
Desirable:
Independent Prescriber qualification or working towards it.
Previous experience in general practice, PCNs, or integrated care systems.
Knowledge of EMIS and other clinical systems.
