South London & Maudsley NHS Foundation Trust (SLaM) is seeking an enthusiastic and motivated Band 7 Clinical Pharmacist to join the Pharmacy team at Lambeth Hospital. This role offers an excellent opportunity to deliver high-quality clinical pharmacy services across inpatient wards and multidisciplinary teams within the Lambeth directorate. The successful candidate will play a key role in promoting the safe and effective use of medicines, supporting medicines optimisation, and contributing to patient-centred care within a leading mental health Trust.

What you can add:

Ensure the safe, effective and legal use of medicines at ward and dispensary level, in accordance with Trust policies, clinical guidelines and statutory requirements. Provide clinical pharmacy input as part of the multidisciplinary team, including medicines reconciliation, discharge counselling, inpatient consultations and patient medication education. Support and work collaboratively with Medicines Management Technicians to optimise medicines use and facilitate safe discharge planning. Review prescriptions for appropriateness, safety and legality, resolving or escalating actual and potential medicines-related issues, including controlled drugs. Contribute to reducing medicines-related risks by promoting safe medication practices, identifying and reporting errors, and supporting the Trust's medicines safety agenda.
What you'll need:

Minimum of 6 months' post-registration experience working as a Pharmacist in an NHS or private hospital setting within the last 2 years. Registered with the General Pharmaceutical Council (GPhC). Master's degree in Pharmacy (MPharm or equivalent).
