We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will possess a strong background in patient care and medication management, with a particular emphasis on ICU experience. This role is crucial in ensuring the safe and effective administration of medications, as well as providing comprehensive pharmaceutical care to patients.
Responsibilities
Dispense medications accurately and efficiently, ensuring compliance with legal and regulatory requirements.
Provide expert advice on medication administration, potential side effects, and interactions to both patients and healthcare professionals.
Conduct thorough medication reviews for patients, particularly those in intensive care units, to optimise therapeutic outcomes.
Collaborate with physicians and other healthcare providers to develop and implement patient-specific medication plans.
Maintain accurate records of prescriptions and patient interactions, ensuring confidentiality and compliance with data protection regulations.
Educate patients on proper medication use, including dosage, timing, and storage instructions.
Stay updated on the latest developments in pharmacology, therapeutics, and relevant clinical guidelines.
Requirements
A valid pharmacy degree from an accredited institution and current registration with the relevant regulatory body.
Proven experience in an ICU setting is highly desirable.
Strong knowledge of anatomy and physiology as it relates to pharmacotherapy.
Excellent communication skills with a focus on patient care and education.
Ability to work collaboratively within a multidisciplinary team while demonstrating leadership when necessary.
