To provide safe, effective, and patient-centred pharmaceutical care in accordance with professional standards, ensuring the
safe supply, management, and use of medicines
across the assigned healthcare setting (hospital, community pharmacy, or specialist unit).
Key Responsibilities:
1. Clinical Services:
Review and validate prescriptions for accuracy, legality, and appropriateness.
Provide
clinical advice
to healthcare teams on medication therapy.
Participate in
ward rounds
or multidisciplinary team meetings (hospital settings).
Monitor patient outcomes and
report adverse drug reactions
.
2. Dispensing & Supply:
Accurately dispense and supply medicines in line with
GPhC standards
.
Maintain
controlled drugs records
and other legal documentation.
Ensure
timely stock management
and ordering to maintain service continuity.
3. Medicines Management:
Support
formulary compliance
and cost-effective prescribing.
Contribute to
medication safety initiatives
, audits, and incident reporting.
Maintain patient medication records accurately.
4. Education & Training:
Provide advice and training to pharmacy staff, students, and other healthcare professionals.
Keep up-to-date with
clinical guidelines, legislation, and best practices

5. Compliance & Governance:
Adhere to
GPhC Code of Conduct

Participate in quality assurance and risk management activities.

Person Specification / Essential Skills:
Registered Pharmacist
with the
General Pharmaceutical Council (GPhC) UK
.
Strong clinical knowledge and patient-focused approach.
Excellent communication and interpersonal skills.
Competent in IT systems for prescribing, dispensing, and record keeping.
Ability to work independently and as part of a team.
Knowledge of
UK medicines legislation, NICE guidelines, and local formularies
.
Desirable / Specialist Skills:
Experience in
hospital pharmacy, aseptic preparation, or clinical specialities
depending on the setting.
Hospital pharmacists may work in wards, pharmacy departments, or aseptic units.
Community pharmacists may work in retail or NHS community settings.
