Mersey and West Lancashire NHS Foundation Trust is seeking a highly motivated and experienced Band 8a Clinical Pharmacist to join our dedicated team at Southport and Formby District General Hospital. This is a fantastic opportunity for an enthusiastic pharmacist to play a key role in delivering high-quality clinical pharmacy services and optimising medication management for inpatients.

About the Role

As a Band 8a Clinical Pharmacist, you will provide expert pharmaceutical care to wards and departments, ensuring the safe and effective use of medicines. You will work closely with medical and nursing teams to improve patient outcomes and enhance the quality of prescribing.

Your key responsibilities will include:

Taking

drug histories on admission

and completing

medicines reconciliation

for inpatients. Screening newly admitted patients for

medicines optimisation issues

and developing individualised care plans. Conducting

medication reviews

to ensure safe, evidence-based, and cost-effective prescribing. Monitoring and adjusting patient medication based on

therapeutic drug monitoring results

and response to treatment. Counselling patients on their medication, ensuring they understand their drug therapy and any potential side effects. Providing

professional and legal clinical supervision

in the dispensary, ensuring best practices in dispensing and medicines management.
This role offers a unique opportunity to contribute to patient safety, effective prescribing, and the continuous development of clinical pharmacy services within the trust.

What We're Looking For

To be successful in this role, you will need:
Essential Criteria:

A

master's degree in pharmacy

(MPharm) or equivalent qualification. Registration with the

General Pharmaceutical Council (GPhC)

. Evidence of

ongoing Continuous Professional Development (CPD)/Revalidation

in line with professional requirements.

Previous relevant hospital experience

in a clinical setting. A strong foundation in

Clinical Pharmacy and Medicines Optimisation

. Excellent

verbal and written communication skills

to liaise effectively with multidisciplinary teams. Strong

time management, decision-making, and organisational skills

. Computer literacy and experience using

clinical systems and electronic prescribing tools

.
Desirable Criteria:

A

postgraduate qualification

in Clinical Pharmacy or an independent prescribing qualification. Experience in a

specialist clinical area

such as acute medicine, critical care, or cardiology. Previous experience supervising and mentoring junior pharmacists or pharmacy technicians. Familiarity with NHS policies and procedures in medicines management.
