We are seeking a dedicated and knowledgeable Pharmacist to join our healthcare team. The ideal candidate will play a crucial role in ensuring the safe and effective use of medications for patients. This position requires strong clinical skills, attention to detail, and a commitment to patient care. The Pharmacist will collaborate with healthcare professionals to provide comprehensive pharmaceutical care, manage medication therapy, and educate patients about their medications.
Responsibilities
Review and interpret physician orders to ensure accuracy and appropriateness of prescribed medications.
Prepare and dispense medications, including compounding when necessary, while adhering to aseptic techniques.
Conduct patient assessments to monitor medication efficacy and safety.
Provide patient education on medication administration, potential side effects, and drug interactions.
Maintain accurate records of medication inventory management and ensure compliance with HIPAA regulations.
Collaborate with healthcare teams in acute care settings, providing critical care support as needed.
Administer immunizations and manage pain management therapies for patients.
Utilize pharmacy software, EMR systems, and EHR systems for efficient patient care documentation.
Participate in clinical pharmacy activities, including medication therapy management and patient service initiatives.
Skills
Strong knowledge of medical terminology, anatomy, physiology, and pharmacology.
Experience in LTC pharmacy or hospital settings is preferred.
Critical care experience is highly valued; pharmacy residency or clinical pharmacy experience is a plus.
Proficient in IV insertion and IV infusion techniques.
Familiarity with compounding medications using aseptic techniques.
Excellent communication skills for effective patient interaction and education.
Ability to assess patient needs and provide tailored pharmaceutical care.
Knowledge of inventory management practices within a pharmacy setting.
