Note:

Laurel Bank Surgery is merging with the Village Surgeries Group on 1st April 2026. The successful candidate must be flexible to work from any of the sites in Malpas, Tattenhall and Farndon.

Job Summary



Manage systems and processes in an efficient manner to maintain and improve the quality and safety related to medicines in the practice. The role holder will handle medication queries, implement medication changes, review and update patient medical records from the hospital discharge letters and support the wider medicines management of the practice. The role holder will support the Clinical Pharmacist, Dispensary Manager and Prescribing Lead GP as required.

Main Responsibilities



Medication queries



Handle the medication queries on a daily basis, interrogate the patient record, and provide as much information as possible for the doctor in their decision making process. Contact the patient for more information if required to find out the patient's story. Stop duplication of requests. Deal with patient telephone calls and tasks regarding medication related queries and actions. Be a conduit between the Doctor, patient and Dispensers.

Medication information/changes from all hospital letters.



Review & implement actions and medication changes on discharge letters. Reconcile the medication. Contact the patient to discuss as necessary.

Practice Point of Contact for Medication



Be point of contact at the practice for patients, pharmacists and Nursing Home Staff.

Safety Alerts



Process Medication Alerts in TeamNet as per policy.

PCN Network DES Quality Improvement Schemes - as per scheme info



Support the Doctors with QOF & long-term conditions work as per the contractual specifications. Provide support to the clinical pharmacist in their role with regards to clinical administration.

ICB Practice Prescribing Scheme



Working with the Prescribing Lead GP and clinical pharmacist, understand the requirements of the prescribing scheme, carry out NHS BSA reimbursement calculations and report on financial aspects of the projects. In conjunction with others carry out the projects agreed by the Partners.

Medicines Formulary management



Maintain up to date knowledge of the local formulary. Manage medication changes as directed by the Dispensary Manager with the approval of the Prescribing Lead GP.

EMIS



Develop in-depth knowledge of EMIS web system. Provide Audit information using EMIS searches and reports as required by the GPs and other staff.

PATCHs



Action any online consultation PATCHs requests received from Reception.

Personal Development:



Contribute to your own and others personal development in relation to duties and responsibilities. Build your knowledge of how other parts of the NHS fits with Dispensing.

Pay: From 13.00 per hour

Expected hours: 12 per week
