Job Description
Main Responsibilities:
Completing medication queries, medicines reconciliation, and repeat prescription processes. CQC Safety searches and medication recalls Liaise with patients regarding medication queries, usage, side effects, and adherence. Ensure the safe and cost-effective use of medicines in line with current formularies and local guidance. Monitor prescribing patterns and support audits related to medicines optimisation and safety. Work closely with GPs, Clinical Pharmacists, Practice Nurses, and other staff to improve prescribing efficiency. Help manage repeat prescription systems. Educate patients on self-care and appropriate medication use.
Manage areas of medicines optimisation framework
This position is not remote, and work will be undertaken within the Practice.

Pay: From 14.09 per hour
Expected hours: 37.5 per week
