Job Description

We are seeking a motivated and professional Pharmacy Technician to join our practice team. The postholder will support safe, effective, and efficient prescribing and medicines use, working closely with GPs, pharmacists, nurses, and other members of the multidisciplinary team.
Main duties of the job

  • Medicines reconciliation following hospital discharges and care transfers.
  • Processing and managing repeat prescriptions in line with practice policy.
  • Providing advice to patients on medicines and supporting compliance.
  • Supporting long-term condition reviews under pharmacist/GP supervision.
  • Liaising with community pharmacies, hospitals, and care homes to ensure continuity of care.
  • Supporting prescribing safety audits, QOF, and quality improvement initiatives.

Job Title: Pharmacy Technician
Reports to: Clinical Pharmacist, GP Partners, Practice Manager
Location: GP Surgery
Job summary
To work as part of the general practice team to optimise medicines use, support safe prescribing, and improve patient care. The pharmacy technician will play a vital role in medicines reconciliation, repeat prescription management, and patient education to ensure safe, effective, and efficient use of medicines.
Key Duties & Responsibilities
Carry out medicines reconciliation following hospital discharge, outpatient clinics, or care home transfers.
Support repeat prescription processing, ensuring appropriate reviews, accuracy, and cost-effectiveness.
Support pharmacist with project work
Review patients compliance with treatment, encouraging safe and effective medicines use.
Provide information and advice to patients, carers, and colleagues on medicines and minor ailments.
Carry out housekeeping whilst in patient records e.g. dose optimisation and medicines synchronisation.
Assist with chronic disease management by supporting long-term condition reviews under the supervision of a pharmacist/GP.
Work closely with GPs, practice nurses, and pharmacists to ensure safe prescribing and reduce medicines-related risks.
Support medication safety initiatives, audits, and quality improvement projects.
Ensure prescribing follows national guidance (e.g., NICE, local formularies) and practice policies.
Contribute to repeat prescribing reviews, reducing waste and promoting cost-effective use of NHS resources.
Liaise with community pharmacies, hospitals, and care homes to ensure continuity of care.
Participate in training, supervision, and continuous professional development.
Participate in practice meetings, patient participation groups, and other meetings to improve engagement of the role of pharmacy technician
Ensure that all major MHRA drug alerts, where appropriate and within level of competence, are acted upon within practices over an appropriate time frame and to advise on any medication changes that occur as a result.
Contribute to the achievement and maintenance of good to outstanding CQC registration status.
