Kingston GP Chambers (GP federation) are seeking to recruit a talented Pharmacy Technician.
In this role, the successful post-holder will:
Support the GPs & PCN Clinical Pharmacists in the delivery of clinical pharmacy services
Perform medicines improvement work to maximise safe, cost-effective best practice in prescribing under the supervision of the GP or Clinical Pharmacist.
Assist with medication safety monitoring systems (e.g. high-risk drugs)
Liaise with clinicians and administrative staff to resolve medicine-related queries
Monitor practice prescribing and adherence to practice, local and national prescribing policies
Job responsibilities
Issuing prescriptions in line with agreed clinical protocols and within your competency
Processing repeat medication requests, including the conversion of acute requests to repeat medicines, changing quantities of medicines against local agreed clinical protocols
Working with practice team to develop safe, effective and efficient systems for repeat prescribing
Medicines reconciliation post discharge and on other transfers of care
Providing a contact for patients and colleagues with medication queries
Supporting the PCN practices to deliver on QIPP agenda, QOF, KPIs and locally commissioned enhanced services
To update and maintain accurate patient medication records on the practice clinical computer system, including advice given and action taken
Liaising with secondary and primary care colleagues to ensure correct medicines are continued following the transfer of care.
Monitoring practice adherence to the repeat prescribing policy
Assist the GP/Pharmacist with data analysis of prescribing behaviour in line with NICE guidance and local clinical commissioning directives.
Assisting with the development and review of medicine audits
Project work/prescribing initiatives under the support of the GPs & Pharmacist
To support those with chronic diseases to develop good lifestyle changes to assist in the management of their conditions
Support the GPs & Clinical Pharmacists in Structured Medication Reviews (e.g. organise necessary monitoring tests before SMR)
Assist with the development and implementation of systems to ensure safe and effective monitoring of high-risk medication
Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing
Review and analyse prescribing data across the network using available tools producing reports and feedback for practices
Undertake, participate and share outcomes of clinical audits to facilitate improvement
Review changes to medication that result from CAS/MHRA alerts, product withdrawal and other local/national guidance
Work collaboratively with the PCN team
Develop relationships and work closely with community, hospital and MMT pharmacy colleagues and the wider health system
Participate in practice meetings, patient participation groups and other meetings to improve engagement with the role of pharmacy technician
Experience
Previous experience as a Pharmacy Technician is preferred but not essential; training will be provided for the right candidate.
Strong understanding of medication administration practices and patient care principles.
Excellent communication skills, with the ability to interact effectively with patients and healthcare professionals.
Attention to detail and strong organisational skills are essential for managing multiple tasks efficiently.
