to join our GP practice team. The successful candidate will support safe, effective, and efficient use of medicines across the practice, working closely with GPs, nurses, clinical pharmacists, and administrative teams. This role is patient-facing and administrative, requiring strong communication skills and a commitment to improving patient care through better medicines management.
Key Responsibilities:Medicines Management & Clinical Support
Assist in medication reviews and reconciliation, especially post-discharge or after outpatient appointments.
Manage repeat prescriptions, ensuring accuracy and compliance with prescribing guidelines.
Identify and action prescribing issues under the guidance of a pharmacist or GP.
Support long-term condition management through medicine optimisation activities.
Provide advice to patients on medication use and adherence (within competence).
Help implement prescribing protocols and formulary changes.
Administrative Duties
Liaise with community pharmacies regarding prescription queries.
Process electronic prescription service (EPS) requests and queries.
Maintain accurate records of medication reviews and interventions.
Assist with audits, data collection, and quality improvement initiatives related to medicines use.
Contribute to QOF, CQC, and other practice compliance requirements.
Collaborative Working
Work closely with GPs, practice nurses, clinical pharmacists, and reception staff.
Participate in multidisciplinary team meetings to discuss patient care and medicines optimisation.
Contribute to staff training regarding repeat prescribing and medicines safety.
Essential Criteria:
Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC).
Demonstrable experience in a pharmacy setting (community, hospital, or primary care).
Strong understanding of medicines management and repeat prescribing systems.
Excellent communication, interpersonal, and IT skills.
Ability to work independently and as part of a multidisciplinary team.
Desirable Criteria:
Experience working in a GP practice or primary care environment.
Knowledge of EMIS Web or other GP clinical systems.
Understanding of QOF, CQC, and NHS priorities in primary care.
Additional qualifications or CPD in medicines optimisation or primary care services.
