Our patient list is growing and we would really value some support with all aspects of medication. This would involve speaking with patients, helping our team of clinicians and playing a role in developing our medicines management services. Minimum Role Requirements:
Our pharmacy technician must be:
a. as registered with the General Pharmaceutical Council (GPhC);
b. meets the specific qualification and training requirements as specified by the GPhC criteria84 to register as a Pharmacy Technician;
c. Enrolled in, undertaking or qualified from, an approved training pathway. For example, the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH);
d. Is working under appropriate clinical supervision to ensure safe, effective and efficient use of medicines in order to deliver the key responsibilities outlined in section B2.2. B2.2 in the DES Contract.
OVERVIEW OF THE ROLE
Registered Pharmacy Technician with GPhC who is able to perform a variety of functions in pharmacy while working under the supervision of the clinical team at Hampton Surgery.
The registered Pharmacy Technician will work in accordance with local policies and alongside other members of the MDT to help manage and optimise patient's medication. Registered Pharmacy technicians work in accordance with GPhC standards set out for pharmacy technicians and will work within their professional boundaries.
JOB SUMMARY:
The role is to support the practice to ensure the delivery of safe, effective and efficient systems for repeat prescribing, medicines optimisation, reducing medicines waste and maximising patient outcomes.
The post holder will work as part of a multi-disciplinary team and support all Practice staff to develop and improve processes. Innovative ideas are welcomed, and excellent communication skills, enthusiasm and motivation are essential skills for a successful candidate.
The roles involves supporting the surgery, improving and ensuring medicines optimisation processes are being followed through various duties which include daily medication requests and queries, and communicating with patients and Community Pharmacies.
Working hours and location/s to be agreed with the selected candidate. Access to all Practice clinical computer systems is available from each Practices site to enable remote working
MAIN DUTIES / RESPONSIBILITIES:
Ensure effective medicines use, through shared care decisions.
Medicines optimisation including medicines administration (e.g. inhaler technique), medicines reviews and medicines reconciliation.
Support the clinical pharmacist with SMR (Structured medication reviews)
Support medicine reviews and reconciliation for new patients (and new care home patients) and synchronising medicines.
To liaise where necessary with community pharmacists and other pharmacist in relation to medicines optimisation.
Counselling patients to use their medications effectively and offer lifestyle advice and other service information as appropriate.
Managing shared care protocols.
Support initiatives for antimicrobial stewardship.
Support the MDT to ensure medicines optimisation processes are being adhered to.
To help plan, develop and support the introduction and development of new working processes within the practice to optimise the quality of prescribing as well as the ordering processes and return processes (reducing medicines wastage).
Provide training and support.
Promote eRD (electronic repeat dispensing) and online ordering.
Develop and maintain professional relationships with members of the MDT.
To lead the practice in the auditing, monitoring and prescribing of high risk drugs.
Support the practice reception team with general prescription queries and referring as appropriate.
Support the implementation of national prescribing policies and guidance within GP practices, care homes and the wider PCN.
Undertake clinical audits to support quality improvement measures and contribute to the delivery of the DES (Direct Enhanced Service)
Support the practice/ PCN in achieving the Quality and Outcomes Framework (QOF) targets, Network DES IIF targets and any targets from other organisations.

Participate in research and utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required
In partnership with other clinical teams, collaborate on improving the quality of healthcare responding local and national policies and initiative as appropriate, work effectively with individuals in other agencies

Co-operate and participate as required in any research projects as identified
SUPERVISION
The post holder will have access to appropriate clinical supervision and an appropriate named individual in the surgery or PCN to provide general advice and support on a day to day basis

To assist the partners with investigating untoward incidents or near misses in accordance with the internal Significant Events
LIAISON
Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines, with appropriate regard to confidentiality

To demonstrate professional and ethical behaviour always including when liaising with internal and external colleagues
HEALTH & SAFETY
The post holder will contribute to, and implement, a full range of promotion and management of their own and others health and safety and infection control as defined in the practices Health & Safety Policy, the practices Health & Safety manual, and he practices Infection Control Policy and Published Procedures.
The post-holder will comply with all necessary training requirements relevant to the role as identified by the organisation
The post-holder must comply at all times with the organisation and Practices Health and Safety policies, in particular by following agreed safe working procedures and reporting incidents using the organisations Incident Reporting System

In the performance of the duties outlined in this , the post-holder may have access to confidential information relating to patients and their carers, practice staff and other healthcare workers. They may also have access to information relating to the practice as a business organisation. All such information from any source is to be regarded as strictly confidential

You must make yourself aware of your responsibilities to maintain the wellbeing and protection of vulnerable children and adults.
