Band 6 Physiotherapist - Croes Atti D2RA Unit



Croes Atti is a brand-new 14-bedded Discharge to Recover and Assess (D2RA) unit, based within a Flintshire Social Services care home in Flint. This innovative service supports individuals who have been discharged from hospital, providing a reablement-focused approach to care and therapy that promotes independence and helps people regain essential skills before returning home.



As the Band 6 Physiotherapist, you'll be part of a well-supported, collaborative team, working alongside Social Services colleagues including Social Workers, Occupational Therapists, and Technical Instructors. This is an exciting opportunity to implement a proven D2RA model in a new setting, shaping the service and making a real difference to patient outcomes.



Your clinical caseload will be varied and rewarding, including orthopaedics, neurology, and frailty, and you'll work closely with the wider Flintshire community physiotherapy teams--covering domiciliary, reablement, crisis response, and the established D2RA unit at Marleyfield.



We offer a strong supervision structure, a well-established in-service training programme, and support from Clinical Specialists to help you develop and thrive. There will also be opportunities to contribute to service development and enhance your leadership skills.



Please note:

There is an expectation to participate in the 7-day Flintshire CRT rota.



To carry out physiotherapy assessments and treatments; developing individualised therapeutic treatment programmes for a specialised caseload in the relevant clinical area. Patients may include those with physical and psychological problems in area of speciality; some of whom may have a level of complexity. Team leader; clinical specialist or advanced practitioners are available for clinical and managerial support.



To ensure a high standard of care for each patient working with patients, families or carers, multi-agency teams and disciplines involved.



To communicate with these patients, carers, agencies and other disciplines in an appropriate manner, often with sensitive information, diagnoses or outcomes.



To supervise, teach and appraise peers, physiotherapists, physiotherapy assistants, technical instructors, students and other members of the multidisciplinary or multiagency teams.



To train and supervise patients, families or carers and staff from other disciplines or agencies to carry out programmes of care devised by the therapist.



To deputise and support the Clinical Team Leader as required in the daily management of the designated team.
