The Trust are looking for a practitioner with the clinical skills and expertise and knowledge to support the pathways that sit under the Intermediate Care service: Community physio, Falls, Reablement /Urgent Community Response/Virtual Frailty Ward. The individual will work within the multi-disciplinary integrated team to jointly assess and plan the care of people who are referred to intermediate care services.
This role involves 7 days working within a shift pattern across an integrated health and social team. The team includes Physiotherapists, Occupational Therapists, Therapy Assistants, Triage Coordinators and Therapy Assistants. You will be working in close liaison with District Nursing, Urgent Community Response teams, Local authority colleagues, Care agencies, Older Peoples Mental Health teams, Specialist services and voluntary services.
Responsibilities of this job role
Be highly skilled in using and reporting on standardised and non-standardised Physiotherapy assessment to diagnose and analyse highly complex patient needs in the context of activities of daily living.
Devise Physiotherapy treatment plans and goals in collaboration with the patient, relatives, carers and the interdisciplinary team.
Assess and issue equipment as appropriate to meet the patients' needs and ensure that up-to-date written and electronic records, reports and activity data are maintained in accordance with professional and local standards.
Be able to create, evaluate all care plans implemented and should have a sound knowledge in discharge planning and ability to make further referrals in to the community as required.
Use advanced skills in analysing, selecting and grading activity to achieve therapeutic goals.
To be successful in applying for this role you will need:
Physiotherapy Qualification and experience mandatory
HCPC registration mandatory
RiO training desirable
Driving Licence preferable as this is a Community post
In addition to experience, all Qualified roles will be required to provide evidence of
Relevant Qualifications
Registration with the relevant Registration Body if applicable.
