Job overview




An exciting opportunity has arisen for an enthusiastic and creative Band 5 Physiotherapist to join North Kerrier community therapy teams.



We are looking for a flexible, reliable person who enjoys working as part of a wider multi-disciplinary team.



The community post will involve working in a variety of areas including Home First, Community Rehabilitation Team and CATU to gain experience across multiple teams and services in North Kerrier.



You will be working with a complex and varied caseload, covering a wide variety of conditions including, orthopaedics, MS, MND, Parkinson's, amputees, falls and frailty. Every visit and treatment plan are different, bringing variety and challenge to each working day.



You will be very well supported by our experienced and highly skilled team of Band 6 and 7 therapists and nurses across all teams, with opportunities for joint work and shadowing more specialist services, attending our monthly in service training. We full encourage and support training for further development identified in regular supervision and appraisals.



We are hoping this will form part of a wider Band 5 rotational scheme in the future - aiming to start with Newquay inpatient therapy team


Main duties of the job




Assessing new patients on the inpatient unit and in the patient's homes including residential and nursing homes. There is also opportunity to bring those patients into a hospital outpatient setting to be seen if appropriate. Setting treatment goals and following out treatment plans. In reach into community hospital to facilitate early discharge into the community and to continue rehabilitation in the community setting. Working with reablement services such as STEPS Admission avoidance Liaising with the wider multidisciplinary team to enable the patient to reach their goals

Degree or Diploma in Physiotherapy HCPC Registered IELTS unless qualification taken in English Experience of working within a multidisciplinary team and an understanding of roles
