We require a dynamic and enthusiastic Physiotherapist to work in the community, supporting patients in the Leatherhead area in the picturesque Surrey Downs, for a fixed period of 12-months.



The successful applicant will be employed by Epsom & St Helier NHS Trust and to work in Leatherhead Primary Care Network, as part of the Community Therapist Team. The PCN works within Surrey Downs Health and Care, a partnership between three GP Federations, community trust and acute trust, with the aim of delivering true transformational change for adult community services across Surrey Downs, to the benefit of patients and the local healthcare system.


Main duties of the job




To work as a key member of the multidisciplinary team in the assessment (including Comprehensive Geriatric Assessment) and management of a caseload of clients whose needs are best met in their own home or community setting. To plan, deliver and evaluate the treatment needs of clients with a variety of complex social and health care needs. To participate as an active member of the multidisciplinary team in developing and delivering high quality and innovative services to the local population. To promote effective teamwork delegating activities appropriately, to the benefit of the patient and service delivery. Responsible for the provision of Physiotherapy service for Surrey Downs Health and Care. To liaise closely with other health and social care professionals.
To contribute to audit, implementation of best practice guidelines and contribute/participate in in-service training

Detailed job description and main responsibilities




To perform advanced physiotherapeutic assessment of the patient with complex presentations using advanced clinical reasoning skills to create targeted treatment plans using SMART goals specific to the patient's condition. To formulate and deliver an individual or group physiotherapy treatment programme In line with best practice guidelines. To help co-ordinate and ensure effective delivery of therapy services liaising and co-operating with colleagues across other PCN's/Adult social care teams to provide equitable services. To evaluate patient progress, reassess and alter treatment programmes as required and to advise and support more junior colleagues to this effect. To handover treatment plans to the rehabilitation assistant to support with the patient centred goals. Willingness and enthusiasm to work toward cross-disciplinary competencies. To have an awareness of local community services and be able to refer to these appropriately and in a timely manner. To understand and manage clinical risk within own patient caseload and within the service and to seeking guidance from senior staff as required. Work within Epsom and St Helier policies and CSP clinical guidelines and participate on CPD to maintain professional registration. Monitor own and others quality of practice as appropriate. To keep accurate and complete records of work undertaken in line with CSP, Community Therapy Team, Surrey Downs Health and Care standards. Maintaining confidentiality and information governance standards at all times. To be able to assess and identify those who lack capacity to consent to treatment and be able to work with them within a legal framework. To contribute to development of patient care by attending relevant team, departmental or other appropriate meetings and comment on recommended changes to clinical care or service provision.

Pre-graduate experience in community settings (preferably NHS) Experience of working as part of a multi-disciplinary team Previous experience of working as a Junior Physiotherapist #
