Are you a dynamic and compassionate Physiotherapist looking for a rewarding role where you can make a real difference to patients in crisis?We have received significant investment to expand our Urgent Community Response (UCR) service and are now recruiting a Band 6 Physiotherapist to join our dedicated multidisciplinary team.



About the Service



The UCR team provides rapid assessment and intervention to patients in the community who are experiencing an acute health or functional crisis -- helping to avoid unnecessary hospital admissions and supporting people to remain safe and independent in their own homes.



Referrals are received from 999, 111, GPs, and other community services , and the team responds to all referrals within 2 hours . As a UCR Physiotherapist, you will play a key role in delivering urgent therapy interventions.



The service operates 8am-8pm 7 days a week. Shift patterns could include a variety of either earlies (8-4), lates (12-8) or long days (8-8).



You must have a current, valid driver's license with a vehicle for work purposes with business insurance. The service also has the option of driving a small van for work purposes to enable efficiency of service delivery.

Provide rapid response within 2 hours to patients at risk of hospital admission, supporting early intervention and preventing unnecessary admissions.



Undertake comprehensive assessments of acutely unwell patients, often acting as the primary clinician in lieu of GP or Paramedic input.



Deliver holistic therapy interventions to ensure the patient's immediate safety, functional ability, and wellbeing within their home environment.



Manage complex manual handling situations, ensuring patient safety and adherence to moving and handling best practice.



Work as an integral part of a multi-disciplinary and multi-agency team , collaborating closely with medical, nursing, and emergency services to deliver coordinated care.



Signpost and refer patients for ongoing therapy, community support, and preventative services to promote recovery and reduce future risk of admission.



Contribute to service development and quality improvement .

To undertake comprehensive holistic assessments for individuals presenting with an acute crisis in their own homes due to deterioration in their health and wellbeing, to reduce or avoid hospital admission.



To formulate individual treatment plans for patients with complex physical and social needs to ensure immediate patient safety in their own homes.



To work autonomously in a crisis situation and manage clinical risk associated with this.



To provide a safe and specialist alternative to conveyance to hospital within two hours of referral.



To complete onward referrals to specialist teams as appropriate for longer term maintenance and prevention.
