Harrow Rapid Response is a national leader in intermediate care that aims to bring the best of hospital care to a patient's home and therefore avoid unnecessary hospital attendances and admissions.
Most of our referrals are from GPs, London Ambulance Service and Accident and Emergency departments. Attending individually and in pairs, we perform an initial assessment at home, carrying out a holistic assessment of the patient and in some cases starting a treatment plan within 2 hours of referral.
The team can utilise a number of pieces of diagnostic equipment in the home, plus we have access to hospital facilities for various scans when required. We also perform our services in A&E to avoid hospital admissions and facilitate safe discharges.
Harrow Urgent Community Response Team looks to provide the very best patient care, aiming to provide the same care a patient would receive as an inpatient. This is facilitated by having real time access to patient records in the community via laptops.
We are looking for a Physiotherapy Assistant who has a passion for patient care, who wants to develop their skills and push themselves outside the normal boundaries of practice and who has a real drive to make sure their patient gets the best treatment possible. Excellent communication skills are essential.
This post will be within the HUCRT/A&E service.
Key Responsibilities:
Work as a therapy technician in HUCRT, supporting patients to regain and sustain their levels of function
Assess patients, define treatment needs in relation to therapy and carry out this treatment, maintaining clinical records
Undertake the learning of new skills and knowledge to include some nursing, physiotherapy and occupational therapy skills, assessing and planning social care packages in agreement with and on behalf of social services. All these skills are competency led
Work in close partnership with hospital colleagues and external agencies such as Social Services, Clinical Commissioning Group (CCG), Housing Associations, GPs and voluntary organisations. This will involve attending meetings outside the Trust and sharing protocols and care management plans with the CCG and other agencies
Work collaboratively with patients, carers and with colleagues in health and social services to ensure effective service planning, policy and delivery of services to their specific patient group
Be involved with assessment and treatment for patients with a wide range of complex physical, sensory, perceptual and cognitive problems within A&E and community settings. Assessments will also include some inpatient assessments
