POST COVID ASSESSMENT SERVICE



RESPIRATORY PHYSIOTHERAPIST



SECONDMENT / FIXED TERM until 31st MARCH 2026 ( 30 hours per week)



A rare opportunity has arisen for an experienced Physiotherapist to join the Post Covid Assessment Service here at Norfolk Community Health and Care Trust as an internal secondment opportunity until end of March 2025 .



This position is ideal for a Senior Physiotherapist with expertise within the field of long term conditions- ideally post covid syndrome, looking to enhance their clinical skills in this new and developing area. You will part of an airways team working closely with the Speech and Language Therapist and a respiratory Nurse.



You will be a dynamic, forward-thinking individual with a passion to lead & deliver first class care to patients with a wide range of symptoms. You will provide support to the band 6 Physiotherapist in the service in close liaison with the Team Lead and Service Lead clinicians.



You will have the clinical expertise & vision that will facilitate innovative Therapy services & develop high quality care enhancing the patient experience. You will work as a clinical expert & be proficient in assessment & prescribing a plan of care to patients with a wide range of symptoms.

Support Team Lead in the development and maintenance of policies and procedures Lead where required on the development and maintenance of processes and pathways To review referrals, support triage and liaison for patients Develop positive and professional relationships . Supporting the MDT process and lead where required at Post-COVID Assessment Service, review patients' holistic needs, communicating highly complex and sensitive information to referrers, and colleagues, patient and carers as required. Where there are barriers to understanding, such as cultural barriers, hearing impairment, mental capacity impairment and other difficulties in comprehension or language impairments; awareness of reasonable adjustments Be required to use tact and persuasive skills in order to coordinate and plan on-going care, treatment or management with colleagues, for patients. Liaise with GP's, Social Services, inpatient teams, other allied health care professionals To participate and lead in meetings as appropriate. Provide clinical expertise and advice to others Ensure activity data and clinical information is recorded on system one

Support Team Lead in the development and maintenance of policies and procedures within the Post-COVID Assessment Service .



Lead where required on the development and maintenance of processes and pathways for Post-COVID Assessment Service . To review referrals, support triage and liaison for patients referred into Post-COVID Assessment Service and the airways team. Develop positive and professional relationships across a wide range of internal and external groups, agencies and service providers. Supporting the MDT process and lead where required at Post-COVID Assessment Service, review patients' holistic needs, communicating highly complex and sensitive information to referrers, and colleagues, patient and carers as required. Where there are barriers to understanding, such as cultural barriers, hearing impairment, mental capacity impairment and other difficulties in comprehension or language impairments; awareness of reasonable adjustments. Use risk stratification methods in partnership with other professionals to identify those patients most likely at risk of health, social care and wellbeing crisis and focus support to ensure the patients independence is maximised. Identification and awareness of inequalities in health outcomes and access to support for Norfolk and Waveney citizens. Be required to use tact and persuasive skills in order to coordinate and plan on-going care, treatment or management with colleagues, for patients. Liaise with GP's, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately. To participate and lead (where appropriate) in multi-disciplinary / multi-agency meetings as appropriate. Provide clinical expertise and advice to others, including social services, GP's and other agencies. Ensure activity data and clinical information is recorded on system one and other tools as required, to be completed on a daily basis. Demonstrate dexterity and co-ordination when using specialist equipment and fine tools, advances sensory skills, manual and mobilising skills.

The post holder will:



Be required to undertake risk assessments and act upon them appropriately Be responsible for ensuring workload for self and others in planned and prioritised according to service targets and needs Ensure own paperwork and records are maintained, and those maintained by the team Contribute to the completion of the staff rota on a monthly basis Contribute to the supervision of the workload demand and resource utilisation within the team Order equipment and supplies via the electronic ordering systems used within the team Actively contribute to the multidisciplinary team meetings and other meetings where appropriate, and chair meeting where required Develop and maintain effective working relationships and communication with all members of the organisation, other professionals, and agencies Instigate, and actively participate in the development of the operational strategy and project management Ensure effective use of resources, both financial and human, by members of the team Participate in the recruitment process, using the opportunity to skill mix to meet the demands of the service Attend in-house training as required to develop current job role Will be able to make own travel arrangements as required Be responsible for safe use and maintenance of equipment and supplies Remain accountable for own professional actions as determined by professional bodies Participate in evidence based practice, using research methodology Ensure robust induction programme for students and new starters Be accountable for work delegated to others
