PRACTICE NURSE

REPORTS TO:

PARTNERS (Clinical) & PRACTICE MANAGER (Organizational / Administrative)

SUMMARY OF THE POSITION
To provide clinical nursing service and support to other professionals and to patients of the practice including treatment, screening, chronic disease management and advice within the professional competence of the post holder. Support and work effectively within the clinical team.

All duties will be carried out in accordance with the NMC Code of Professional Conduct and with regard to the various practice protocols. The nurse is part of a nursing team responsible for ensuring that the practice provides an optimum service of nursing care relevant to the practice population.
MAIN RESPONSIBILITIES OF POST

Meeting patients' nursing care needs as identified, or where care has been transferred to her by a GP.



Initiating and maintaining health screening and promotion clinics for identified areas of the practice population, recognising where modification of systems or referral is necessary. Managing the nursing care of patients with chronic disease, employing personal recall systems suitable to the practice needs. This includes using the practice computer systems for input of information for recall and screening purposes. The following clinics are offered at Pak Health Centre by practice Nursing Team:
Asthma / COPD

Diabetes

Hypertension

Cardiovascular Disease

Travel Vaccination Clinic

Cervical Smears

Amending or adding to services as needs are identified.

Maintaining efficient liaison with all areas of the Primary Care Team and other agencies to assure continuity of care where necessary. Routine treatment in general clinics, including:
Wound management

Blood tests

ECGs

and other general Treatment Room skills, including:

Clinical Nursing Practice Wound Management: Undertake dressings Suture and clip removal Screening Undertake screening including: BP checks, height, and weight, BMI, Urinalysis, smoking status. Carry out New Patient checks Undertake venepuncture according to practice protocol Health Promotion: Make basic assessment of needs of patients Provide nutritional advice on:

Weight

CHD Diabetes BMI Understand patients motivation to adapt their lifestyle Offer lifestyle advice on smoking and exercise and refer as appropriate Vaccinations and Immunisations Administer injections under an individualised prescription or Patient Group Directive. To continually update your knowledge of the implications of immunisation and contra indications Ensure safe storage, rotation and disposal of vaccines and drugs. Ordering of vaccination including childhood and seasonal influenza vaccination Infection Control Lead Support patients and colleagues in adopting sound infection control measures. Apply infection control measures within the practice according to local and national guidelines. Act as a role model promoting a culture where all care is provided in clean, safe environments, excellent standards of clinical care are attained and maintained and all service users, colleagues and visitors are treated with dignity and respect Use clinical knowledge, judgement and skill to influence and embed safe, evidence-based practice in infection control across areas of responsibility. To provide expert advice at an operational level to support providers of health and social care services in infection prevention and control, and to ensure effective monitoring and reporting on programme progress. Manage, monitor, analyse, review and evaluate the delivery of the COVID-19 action plan and the overarching Infection Prevention Delivery Plan to ensure that actions are completed within agreed timescales and specific quality measures. Establish and maintain an effective communication with key stakeholders, colleagues and partners relevant to the work being undertaken, developing cultures and processes to address and learn from conflicting priorities and demands. Develop robust project plans with clear objectives, milestones, resource plans and risk management strategies. Develop cultures and processes to address and learn from previous infection related incidents. Look to continuously improve services in area of responsibility, ensuring resources are utilised effectively and efficiently. Remain up to date and compliant with all relevant legislation, organisational procedures, policies and professional codes of conduct in order to uphold standards of best practice This includes:

Universal hygiene precautions

The collection and handling of laboratory specimens Segregation and disposal of waste materials Decontamination of instruments and clinical equipment Reporting and treatment of sharps injuries Dealing with blood and body fluid spillages Responsible to carry out IPC audits Making sure practice follows the IPC policy and make sure staff understands and implement it Emergency Management To be competent in anaphylaxis and resuscitation techniques attend updates when required Women's Health Have a working knowledge of the national and local Cervical Cytology and mammography screening programmes Can discuss breast self-awareness and provide supporting literature to patients General Duties To maintain accurate patient records on the computer To collect data and participate in regular practice audit and research projects when required To log and dispatch specimens to the hospital To assist the Practice Nurses in the provision of Health Promotion and Chronic Disease Management Education To maintain continuous professional development in keeping with the requirements of the practice. To participate in training of other personnel as appropriate, including unqualified staff. To contribute to the preparation of the Practice and Professional Development Plan. Information and I.T. Facilitate good communication systems within the practice with the use of computerised messages. Extrapolate information received by outside agencies and bring to the notice of all clinicians wherever necessary. Undertake all training and attend any computer data input and retrieval meetings deemed necessary by the Practice Manager to manage the clinical system effectively. Patient Liaison Be available to deal with any patient query that may prove to beyond the ability of the medical receptionist, and be aware of limit of own ability. Ensure that all formal complaints are passed onto the Practice Manager for immediate action

Professional Registration
You will be expected to maintain your own annual registration with the NMC.
