The Practice Nurse will work as part of the primary care team, providing high-quality, evidence-based nursing care to the registered patient population. This role involves delivering a wide range of nursing services including chronic disease management, immunisations, wound care, cervical cytology, and health promotion. The nurse will work collaboratively with GPs, other nurses, healthcare assistants, and administrative staff to meet the needs of patients and the practice. Key Responsibilities: Clinical Duties: Provide skilled nursing care and health assessments to patients.
Manage and monitor patients with chronic diseases (e.g., asthma, COPD, diabetes, hypertension). Perform wound care, dressings, suture removal, and compression therapy. Administer vaccinations and injections (including childhood immunisations, travel vaccines, and flu jabs). Conduct cervical smears in line with national screening guidelines.
Provide contraceptive advice and support (as trained/appropriate). Provide advice and support for lifestyle changes, including smoking cessation, weight management, and alcohol use. Participate in infection prevention and control measures and maintain clinical cleanliness standards. Administrative and Team Duties: Maintain accurate patient records using the clinical system EMIS Participate in the call/recall system for chronic disease and screening programs.
Support Quality and Outcomes Framework (QOF) delivery and other enhanced services. Contribute to clinical audits and quality improvement initiatives. Participate in multidisciplinary team meetings and case discussions. Supervise or support junior staff and healthcare assistants as needed.
