Practice Nurse

Job Description

The Practice Nurse will work as part of the primary care team, providing high-quality, evidence-based nursing care to the registered patient population. This role involves delivering a wide range of nursing services including chronic disease management, immunisations, wound care, cervical cytology, and health promotion. The nurse will work collaboratively with GPs, other nurses, healthcare assistants, and administrative staff to meet the needs of patients and the practice. Key Responsibilities: Clinical Duties: Provide skilled nursing care and health assessments to patients.



Manage and monitor patients with chronic diseases (e.g., asthma, COPD, diabetes, hypertension). Perform wound care, dressings, suture removal, and compression therapy. Administer vaccinations and injections (including childhood immunisations, travel vaccines, and flu jabs). Conduct cervical smears in line with national screening guidelines.



Provide contraceptive advice and support (as trained/appropriate). Provide advice and support for lifestyle changes, including smoking cessation, weight management, and alcohol use. Participate in infection prevention and control measures and maintain clinical cleanliness standards. Administrative and Team Duties: Maintain accurate patient records using the clinical system EMIS Participate in the call/recall system for chronic disease and screening programs.



Support Quality and Outcomes Framework (QOF) delivery and other enhanced services. Contribute to clinical audits and quality improvement initiatives. Participate in multidisciplinary team meetings and case discussions. Supervise or support junior staff and healthcare assistants as needed.



Essential Qualifications and Experience:

Registered Nurse with the NMC (Nursing and Midwifery Council). Previous experience in primary care or community nursing. Competent in chronic disease management (e.g., diabetes, asthma). Trained in immunisation, cytology, and basic life support.



IT literate and familiar with electronic health records. Desirable Skills and Attributes: Experience with EMIS Web. Independent prescribing qualification (V300) desirable but not essential. Team player with excellent communication and interpersonal skills.



Ability to work autonomously and manage time effectively. Commitment to professional development and ongoing learning. Additional Information: Enhanced DBS check will be required. Participation in clinical supervision, annual appraisal, and training updates is expected.



Flexibility to cover additional shifts when necessary is appreciated.
