About us

Pickhurst Surgery is based in Bromley, Hayes. We have a growing list size of approx. 8,800 patients and are part of the Hayeswick Primary Care Network. We have an extended clinical team, which includes Clinical Pharmacists, First Contact Practitioners, a Mental Health Practitioner and GPAs, working alongside the GPs (3 Partners, 2 Salaried GPs and 1 Long term Locum GP) and a Nursing team, supported by the Practice Manager and the Admin team. The Practice advocates a friendly and positive working environment and we are looking for an equally friendly, motivated and versatile individual to join our team.

Job Summary:



The role of Practice Nurse is to support Lead Practice Nurse to continuously develop and modernise the practice systems. To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.

Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.

The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health promotion, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

Practice must ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.

The list of duties below is not exhaustive and may be subject to change as deemed necessary.

MAIN DUTIES AND RESPONSIBILITIES



Patient Care



Provide safe, evidence-based, cost-effective, individualised patient care. Refer patients to other agencies/secondary care as appropriate, completing the referral paperwork accurately and liaising with the secretarial team if required Deliver care according to the Practice Local and the National Institute for Clinical Excellence (NICE) guidelines as well as evidence-based care Provide guidance to staff, patients and their families and carers on pathway navigation, including developing links with key community organisations to develop the practice and patient engagement / satisfaction levels, including working towards including populations which have not yet been reached. A willingness to improve patient care by completing required training and mandatory trainings.

Clinical



Assess, plan, develop, implement and evaluate treatment programmes for patients that promote health and well-being. Help the practice develop and deliver nurse led services, including chronic disease management (including for all areas of QOF). This involves, implementing and using protocols and clinical guidelines. Make professionally autonomous decisions for which he/she is accountable and provide safe, evidence based, cost effective, individualised patient care. Make appropriate referrals to other members of the primary, community and secondary care teams. To provide Travel health advice for patient prior to traveling including malaria prophylaxis, safe sex, sun protection, food hygiene, first aid and emergency medication, health insurance and vaccinations. Undertake procedures including: Blood pressure management, urinalysis, peak flow, pregnancy testing, stitch/clip removal, cytology, Intramuscular and Subcutaneous injection and wound management. Provide support and chaperoning to the doctors for examinations and other procedures. Contribute to the practice achieving its PMS contract obligations including quality, enhanced services, Public Health, organisational and access targets. Ensure all data protection requirements are met when gathering, recording and storing patient data. Liaise and maintain good working relationships with all members of the practice team. Work within the multi-disciplinary team within the practice and across the wider health system Be part of the influenza planning and delivering the annual flu vaccination programme for adults and children. To act support the Infection Prevention Control (IPC) lead and to work to the IPC policy providing the implementation of audits, training, support and evidence of compliance to the Lead Practice Nurse. Participate actively in multidisciplinary clinics, patient reviews and multidisciplinary team meetings, as well as coding and recording the MDT meetings in Emis. To maintain accurate clinical records in conjunction with extant legislation Undertake a variety of clinical skills and provide treatment, advice as per speciality and scope of practice. Use agreed protocols of clinical practice and professional guidelines within the primary, community or home care settings. Develop, implement and embed health promotion and well-being programmes for the wider population. Provide a first point of contact within the Practice for patients presenting with undifferentiated, undiagnosed problems, making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan. Participate in and oversee the input into a practice diabetes programme, managing newly diagnosed patients as per national and local guidance. Report and raise concerns related to Safeguarding, accessing advice and support as required. Be conversant with Practice policies for safeguarding, including the assessment of mental Capacity, and consideration of Deprivation of Liberty Safeguards (DOLS) and the application of the principles of Prevent (counter terrorism awareness). Be responsible for clinical performance indicators at the practice, alongside the GP Clinical Lead/Lead Practice Nurse/Practice Manager to ensure the practice delivers all of its contractural and other targets, such as QOF and locally commissioned services, immunisations, vaccinations and screening targets. Work with patients to support compliance with, and adherence to, prescribed treatments. Cervical cytology and ensure failsafe processes are set and well understood by the nursing team. To support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice) To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.) To support Lead Nurse, implement and embed an effective Care Management programme for frail and housebound patients registered at the practice. To support the team in dealing with clinical emergencies Administration Ensure the nursing team is well supported by robust clinical policies and procedures and legal frameworks; To ensure PGDs are in date, current and signed by yourself. Complete reports for CCG and PHE immunisation and infection reports as requested. Routinely, to carryout stock and vaccination ordering and as per LPN instructions. Ensure clinical rooms are well equipped and clinical equipment is safe and is calibrated on an annual basis. Assess effectiveness of care delivery through self and peer review, clinical supervision, benchmarking and formal evaluation. To take part in the CQC inspection and the workload processes to assurance quality improvement activities across the practice. To ensure correct coding and Arden templates are used effectively. To ensure compliance with the practice chaperone policy. Support and participate in shared learning within the practice, in order to improve patient care. To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately Participate in the review of significant and near-miss events applying a structured approach, i.e., root cause analysis (RCA) Help LPN in the planning and implementation of changes within the nursing team, enhancing the provision of services to patients Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation.

Planning and Organisation:



To support the control of clinics/appointments, retrieve records, liaising with patients, staff, other health professionals or outside agencies Sustain the confidence of patients and carers by tracking and evaluating changes in services so that the knowledge remains current and with other members of the team, act as a central point of contact in the surgery for staff to access current information so enabling them to signpost patients to appropriate support services. To maintain filing systems in line with Policies and Procedures Ability to deal with incoming and outgoing post.

Within their sphere of competence, each member of staff is responsible for promoting and safeguarding the welfare of the children, young people and / or vulnerable adults for whom they are responsible or may come into contact with, in the job role.

Assist with the gathering of statistics and information when required. Provide cover for members of the support team during periods of absence. Carry out administrative tasks as delegated by clinicians and non- clinical staff. Any working issues identified should be proactively brought to the attention of the relevant person, or escalated appropriately. Manage and prioritise own workload. To ensure safeguards are robustly followed, in order to minimise errors and reduce risk. Any other tasks allocated by managers.

The post-holder will proactively assist in the promotion of a safe working environment, ensuring their safety and security, and that of others, as defined in the practice Health & Safety Policy, the practice Health & Safety manual, the practice Infection Control policy and other published procedures. This will include:

Using personal security systems and procedures within the workplace, according to practice guidelines. Identifying the risks involved in work activities, and undertaking such activities in a way that manages or mitigates those risks. Using appropriate Infection Control procedures, maintaining work areas in a tidy and safe way, free from hazards. Actively reporting health and safety risks or hazards, or infection risks immediately, when identified. Keeping your own work areas and general / patient areas generally clean and tidy, assisting in the maintenance of general standards of cleanliness, consistent within the scope of the job holder's role. The post-holder will:

Apply practice policies, standards and guidance. Discuss with other members of the team how the policies, standards and guidelines will affect their work. Participate in audits where appropriate. Provide training to other staff as requested by the GP Partners/Management team.

Security:



The security of the practice is the responsibility of all personnel. Staff must ensure they remain vigilant at all times and report any suspicious activity immediately to their line manager. Under no circumstances are staff to share the codes for the door locks to anyone and are to ensure that restricted areas remain effectively secured.

These duties may be changed from time to time, dependent on the needs of the practice. This will be done in consultation with the employee and, if appropriate, training and instruction will be given.

Qualifications

Nursing degree or equivalent experience

Registered Nurse (NMC)

Evidence of immunisation training and assessment of competence in last 2 years

NMC's 'Academic Assessors', 'Practice Supervisors', and 'Practice Assessors accreditations

Post graduate diploma or degree (Primary Care)
