About the Role:

We are currently seeking an experienced and proactive Practice Nurse to join our well-organised, friendly team based in Merseyside. This is a part-time opportunity with flexibility around working days and hours.

You will be responsible for delivering high-quality patient care across a range of clinical services, with a focus on long-term condition management and health promotion.

Key Responsibilities:

Conduct chronic disease management clinics (Diabetes, Asthma, COPD, Hypertension) Perform cervical screening, ECGs, urinalysis, phlebotomy, and wound care Administer vaccinations (routine, travel, and seasonal - adults and children) Carry out suture and clip removal Provide contraception advice Conduct new patient health checks and NHS health checks Promote patient education and healthy lifestyle choices Undertake occasional home visits to review and support housebound patients

Practice Information:

Modern, purpose-built facility with ample on-site parking Rated "Good" in all areas by the CQC High QOF achievement Part of a collaborative Primary Care Network (PCN) Uses EMIS Web clinical system Surgery open Monday to Friday, 08:00-18:30

Additional Responsibilities:

Maintain accurate and timely clinical records Work closely with the HCA and Phlebotomy teams Support safe stock control and infection prevention procedures Contribute to overall patient care and service improvement

Requirements:

NMC registration Previous experience as a Practice Nurse in a Primary Care setting Proficient in managing long-term conditions and performing core nursing procedures Excellent communication and teamwork skills Competence using EMIS Web preferred
