Job Description

Orchard Surgery is looking for an experienced, enthusiastic and motivated person to join our friendly practice team. The ideal candidate will already have at least 2 years of primary care experience as a Practice Nurse.

We are looking for an exceptional nurse with excellent communication and clinical skills, who will work closely with our Nurse Lead and nursing team to provide high quality patient care. We are looking for a nurse who has the ability to care for patients with flexibility and empathy. We offer regular clinical supervision and training to support your development.

We have 5 GPs, 4 paramedics, 3 clinical pharmacists, 2 pharmacy techs, a First Contact Practitioner physiotherapist, social prescriber, care coordinator and an extensive admin and management team.

We are one of two practices in Park and Orchard Primary Care Network in Horsham. Our management team are actively involved in the running of the PCN and our GP managing partner is one of two PCN co-clinical directors. Our vision for our populations is closely aligned and our focus is on improving the care of our frail elderly housebound population and improving access and quality of women's health care in Horsham. The practice has recently been awarded funding to develop and establish a Women's Health Hub. We are working closely with community organisations in both these arenas.

Key points



Orchard Surgery is in the centre of Horsham. Practice parking is available. We are based within a modern, purpose built surgery We are dedicated to providing a high quality service to our patients. We have a wellbeing programme for our staff. We close the front doors at lunchtimes to give staff a chance to take a break or attend training. We are rated Good in our last CQC report and achieve excellent QOF reimbursement. We work closely with community organisations in Horsham to help our patients through support for dementia, ageing, loneliness, improving lifestyle and wellbeing, volunteering, financial and housing concerns. We work closely with Horsham Wellbeing Hub to hold regular women's health information sessions and we run Menopause Cafe Horsham. We run innovative group consultations and are looking at expanding these. We have an active PPG with regular coffee mornings and fund raising events

Key Responsibilities:



Identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition, as appropriate Work with other health care professionals to diagnose, monitor, manage & treat long-term conditions including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs Infection control policies, training and implementation Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols with GP support Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Support patients to adopt health promotion strategies that promote healthy living, and apply principles of self-care, utilising community assets such as the Horsham Wellbeing Hub Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment Participate in planning and implementing changes within the practice and the nursing team

Job Responsibilities:



Provide assessment, screening and treatment services Provide health education advice to patients Participate in organising and coordinating the provision of nursing services for the practice to include the delegation of duties to other practice team members as appropriate Provide nursing treatments to patients in participation with general practitioners or independently, adhering to agreed protocols Provide general and specific health screenings to the practice patients with referral to general practitioners as necessary Advise patients on general health care and minor ailments with referral to GPs as necessary Deliver vaccination and immunisation as required Provide appropriate wound care Supplies and equipment - treatment rooms and other areas Ensure claims are logged with the practice manager Ensure the maintenance of equipment and stock levels in clinical rooms Pathological specimens and investigatory procedures Undertake to support the administrative team in the collection, storage and dispatch of pathological specimens including swabs, smears etc.

Administrative and professional responsibilities



Participate in the administrative and professional responsibilities of the Practice team Ensure accurate notes of all consultations and treatments are recorded in the patient notes and on the clinical computer system as appropriate Ensure accurate completion of all necessary documentation associated with patient health care Ensure collection and maintenance of statistical information required for regular and ad hoc reports, audits and research programmes Attend and participate in practice clinical meetings on a regular basis Assist in the formulation of practice philosophy, strategy and policy and develop appropriate protocols

Research projects and training



Co-operate and participate as required in any research projects within the practice Support student/staff education and training, in particular student nurses, HCAs and those on work experience or going through induction

Liaison



Maintain effective liaison with other agencies and staff concerned with patient care and with all other disciplines within the Practice, with appropriate regard to confidentiality

Professional/Personal development



Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional or practice development, and participate in any training programme implemented by the practice as part of this employment Participation in an annual individual appraisal, including taking responsibility for maintaining a record of own personal and/or professional development Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work including participation in clinical supervision and acting as a positive role model Support the development of others in order to maximise potential

Qualifications



Essential:

Registered with the NMC as a practice nurse Commitment to ongoing learning and keeping up with current best practice

Experience



Essential:

Minimum 2 years' experience working in general practice Confidence with IT; proficiency in Microsoft Word and Outlook Experience of using SystmOne

Knowledge & Skills



Essential:

Competent in general practice nursing duties Confident and competent in long?term condition management Confident to manage acute and chronic wounds and apply appropriate wound?care principles Awareness of safeguarding principles for adults and children Knowledge of risk?management principles
Desirable:

Competent in immunisations, injections, ear examinations, ECGs, urinalysis and other relevant procedures Understanding of patient group directions, policies and procedures related to chronic disease management, health promotion and family planning Experience in specific areas such as family planning, cervical screening or advanced wound management Experience in research or audit activities, with a willingness to contribute to evidence?based practice
