Church View Medical Centre is a well-established GP surgery located in Harpurhey Health Centre in North Manchester, serving approximately 4,500 patients. We are a training practice with a stable partnership of GP Partners. About the Position
We are seeking an experienced and proactive Practice Nurse to join our established training practice team. The successful candidate will be motivated and committed to providing high-quality care, working collaboratively with the general practice team to meet the needs of our patients.
Work Location:
In person at Church View Medical Centre, permanent full-time to accommodate 30-33 hours per week across 5 days, including cover of a smaller sister surgery for 2 days (1.1miles from the main branch). The sister surgery has 2,700 patients and is run in the same manner as the main site.
Main Duties of the Job:
Management of chronic diseases, including monitoring and developing individual care plans in agreement with patients and other health professionals.
Family planning and women's health, including cervical smears.
Travel health advice and vaccinations.
Child immunisations and advice.
Lead on local and national vaccination programmes.
Infection Control Lead
Ensure accurate completion of all necessary documentation associated with patient health care.
Collect data and participate in regular practice audit and research projects.
Maintain own annual registration with the NMC and adhere to the Code of Professional Conduct.
Ensure own revalidation is completed on time to continue smooth working practice.
Check all Emergency Drugs Boxes on a weekly basis and ensure all items are replaced when expired/used.
Check emergency resus equipment on a weekly basis.
Carry out Infection Prevention and Control inspections monthly and produce an IPC Audit on an annual basis, reporting on any infection control issues highlighted throughout the year.
Occasional supervision of nursing and medical students due to being a teaching practice.
Qualifications:
Experience of proactive working in a GP surgery setting.
Proficiency in developing and implementing care plans for patients such as asthma plan, learning disability plan.
Strong patient care skills with a focus on empathy and compassion.
Qualified RGN/RN/NMC Registered.
