You have a unique opportunity to join our high achieving, forward thinking Nursing Team in shaping the future of primary care. We are a friendly, busy, growing GMS practice in Melksham, working from a purpose-built Medical Centre. We have a multi skilled team with a wealth of experience and are looking to welcome another Practice Nurse with all general practice nursing skills and ideally experience in diabetes to our team.
Responsibilities
Conduct patient assessments, including taking medical histories and performing physical examinations.
Administer vaccinations, injections, and other treatments as required.
Monitor and manage chronic conditions, providing education and support to patients for self-management.
Assist in the development and implementation of patient care plans.
Maintain accurate patient records and documentation in accordance with legal and ethical standards.
Provide health promotion advice and education to patients regarding lifestyle changes.
Collaborate with multidisciplinary teams to ensure comprehensive patient care.
Participate in quality improvement initiatives within the practice.
Skills
Strong commitment to patient care with a focus on delivering high-quality nursing services.
In-depth knowledge of anatomy and physiology to inform clinical practice.
Excellent communication skills, both verbal and written, to effectively interact with patients and colleagues.
Ability to work independently as well as part of a team in a fast-paced environment.
Strong organisational skills with attention to detail in managing patient records and documentation.
Empathy and compassion towards patients, ensuring their comfort and wellbeing during consultations.
