Practice Nurse Or Advanced Nurse Practitioner

Job Description

We are seeking a motivated Practice Nurse to provide clinical care to our diverse and sometimes vulnerable patient population. The successful candidate will work closely with our GP, other healthcare professionals, and administrative staff across our two health centres to deliver comprehensive care and ensure excellent patient outcomes. This role is part-time, and the ideal candidate would fulfil approximately 24 hours per week.

Key Responsibilities:



Assisting with chronic disease management (e.g., asthma, diabetes, hypertension). Performing cervical smears and blood pressure monitoring. Providing patient education and advice on health promotion and disease prevention. Triaging patient calls and managing patient concerns appropriately, ensuring timely access to care. Maintaining accurate patient records in line with legal and ethical guidelines. Assisting in implementing clinical protocols and procedures to ensure best practice. Offering support to patients and families, promoting self-management and healthy living.

Skills & Experience Required:



Registered Nurse (RN) with a valid NMC. Previous experience in a primary care or practice nursing environment (preferred but not essential). Proficiency in chronic disease management and general clinical procedures. Strong communication and interpersonal skills. Ability to work both independently and as part of a small, dedicated team. A forward-thinking mindset, with an interest in contributing to the continuous improvement of patient care through innovation and new solutions.
