We are seeking a dedicated and compassionate Practice Nurse to join our healthcare team. The ideal candidate will play a vital role in providing high-quality patient care, assisting with clinical procedures, and ensuring the smooth operation of the practice. This position requires excellent communication skills, a strong commitment to patient welfare, and the ability to work collaboratively within a multidisciplinary team. We are welcoming applicants from newly qualified nurses.
Responsibilities
Deliver high quality, compassionate, evidence based and person-centred nursing care to patients
Provide health education and advice to patients regarding lifestyle changes and disease prevention.
Provide long term condition reviews
Administer medication for patients in accordance with patient group directives (PGDs)
Request pathology services and process pathology results as required
Deliver women's health services (e.g. cervical smears)
Support other members of the practice clinical team with nursing services including supporting clinical emergencies
Recognise, assess and refer/signpost patients to other members of the practice clinical team and/or external partner agencies as required
Support the practice in offering student nurse placement mentoring, coaching and teaching
Liaise with external services/agencies where necessary to ensure the patient is supported appropriately
Safeguard children and vulnerable adults, ensuring that concerns are escalated appropriately in accordance with local and national policies.
Support the Infection Prevention Control (IPC) lead and the implementation of the IPC policy taking part in audits, training and support
Participate in supervision, peer support and professional development arrangements
Participate in continuous professional development activities to enhance nursing skills.
Experience
Registered Nurse (RN) qualification with valid registration with the Nursing and Midwifery Council (NMC).
Proficiency in using electronic health record systems is advantageous.
Strong interpersonal skills with the ability to build rapport with patients and their families.
Excellent organisational skills and attention to detail.
Ability to work effectively under pressure in a fast-paced environment.
