Job Responsibilities The post holder will General Work with nursing colleagues, GPs and other professionals to develop quality standards, protocols/guidelines/patient group directions to ensure safe, appropriate and effective care Work within the latest NMC Code of Professional Conduct Ensure the delivery of nursing care is evidence based and own practice is clinically reviewed through supervision and reflective practice as part of revalidation Provide clinical support to the nursing team and promote collaborative working relationships between all members of the multi-disciplinary team. Contribute to clinical/staff meetings professionally and effectively Achieve targets set thus contributing to the profitability of the practice, by timely completion of paperwork Manage and control effective and economic use and ordering of stock, dressings, drugs etc. monitoring usage and costs Record accurate consultation notes and information in patients records Have knowledge and experience of the smooth running of the treatment room, including general maintenance of equipment, reporting any defects to the nursing manager Attend training courses in line with organisational standards and expectations Ensure all mandatory training is completed and up to date Communicate and present professionally at all times Offer additional support/holiday/sickness cover by arrangement Monitor, interpret and evaluate test results, making appropriate referrals to primary, community and secondary health care teams when necessary Share current and learning and information to all team members in order to share good practice, including future courses and conferences Be able to deal with child protection and vulnerable individuals Takes a lead role in managing at least one designated long-term condition (LTC), after completing structured mentorship and guided training Collaborates effectively within a multidisciplinary team (MDT) to promote integrated, personalised patient care Delivers essential nurse-led services to communities within the PCN Conducts early identification, prevention and ongoing management of cardiovascular disease and other chronic conditions Engages actively in quality improvement projects aimed at boosting patient outcomes and addressing health disparities Provide administration in a timely manner Adhere to best practice recommended through clinical guidelines and audit process Treatment Room Manage nursing emergencies and minor illness Work individually and as part of a team Offer immunisations to children, adolescents and adults, in line with the National Immunisation Schedule Offer injectable therapies to children, adolescents and adults in accordance with patient specific directions Provide vaccination services Assess and provide wound management Perform ECGs Perform venepuncture Insert ring pessaries Take BP and BM Testing Perform cervical screening Offer Family planning interventions (optional) Provide ear syringing Prioritise health issues and intervene appropriately Ability to triage, assess, diagnose and treat or refer on to a suitable member of staff or external professionals Infection Prevention & Control In accordance with the Health and Social Care Act amended 2012, the post holder will actively participate in the prevention and control of infection within the capacity of their role. The Act requires the post holder to attend infection prevention and control training on induction and at regular updates and to take responsibility for the practical application of the training in the course of their work.



Infection prevention and control must be included in any personal development plan or appraisal. I.T. Utilise the EMIS Clinical System to include population manager Apply the principles of read coding and have the ability to descend through code levels to reach most accurate read code level for diagnosis, operation, disease etc. Understand the relevance of read coding to searching Use of systems within the practice including docman, EMIS, outlook/email, telephones, internet Other Undertake effective professional development as necessary to keep up to date with current requirements and in line with Practice policy Develop effective working relationships with the ICB Communicate effectively and professionally with patients and their families, other teams and departments, offering support when appropriate Be flexible, helpful and supportive of other teams and departments Undertake other projects or responsibilities as requested Have an understanding of: Guidance related to Health and Safety Guidance related to patient confidentiality and information in line with Caldicott principles.
