Experienced Practice Nurse with experience of managing Long Term Conditions - Join Our Friendly GP Surgery Team

24hrs per week - to be worked Wednesday - Friday

Salary:

19.78 - 23.82 depending on experience

We are a welcoming and supportive GP surgery at the heart of our community, and we're looking for a caring and motivated

Practice Nurse

to join our team.

This is a great opportunity for a nurse who enjoys building relationships with patients and wants to make a real difference in

long-term condition management

(such as diabetes, asthma, COPD, and cardiovascular health), while also being involved in everyday practice nurse work.

What the role involves

Supporting patients to manage their long-term health conditions with confidence. Carrying out a variety of clinics, including immunisations, cervical screening, wound care, and NHS Health Checks. Helping patients stay well with health promotion and lifestyle advice. Working closely with GPs, nurses, and the wider team to deliver joined-up care.

What we're looking for

We'd love to hear from you if you are:

An experienced Registered Nurse (NMC) Experienced in supporting patients with long-term conditions. Confident with core practice nursing skills such as immunisations, wound care/leg club, and cytology. Someone who values teamwork and enjoys working in a community setting. Confident with staff and patient teaching
