Practice Pharmacist
Job Description
Responsible for supporting the partners in ensuring effective medicine management within the practice, identifying areas for improvement and initiating and managing change. Overarching responsibilities To deliver Medication related patient services as determined by practice policy and local and national guidance. Lead, support and guide the in-house prescription and pharmacy team, in order to maximise safe, cost-effective prescribing and maintain and improve the quality of patient care. Liaise and provide support to both medical and non-medical prescribers in the practice to ensure safe, cost-effective prescribing.
