Job Description

The Virtually Assessed Pathway clinic (VAP) is an award - winning service, unique to Velindre Cancer Centre. The post holder will be joining a small team of nurses and pharmacy technicians as one of the 7 Independent Prescribers who work within the VAP Service. 50% of their job plan will also be in face to face clinics working alongside the Consultant / medical team in their parent clinic. Each IP will be have 2 or 3 cancer sites within their scope of prescribing practice.



The VAP clinic is not just a pre SACT assessment clinic, we manage the patients' cancer pathway and as a result we become part of their journey. We maintain continuity of care and as a result we develop a relationship, assessing the patient before each cycle of treatment for months at a time.



The VAP clinic currently works with the following teams assessing and prescribing for patients on suitable regimens from the following cancer sites Colorectal / Breast / Urology / Gynae / Skin/ Hepatobiliary/ UGI /Lung .



A key aspect of the role includes working as part of site-specific teams in face to face clinics as an independent prescriber (IP) in the SACT pre assessment clinics. Experience in these cancer sites would be beneficial but is not essential.



This job plan will require approximately 80% Clinical and 20% clinic admin or focusing on the remaining practice pillars, leadership and management, education and research.



The VAP service is a virtual pre SACT assessment clinic, telephoning patients 2-3 days before each cycle of SACT is due, clinically assessing their fitness to proceed, interpreting the blood results and prescribing the SACT. The Pre SACT assessment is usually undertaken by a Band 5/6 very experienced Nurse or Pharmacy Technician and the IP prescribes based on that assessment.



Velindre Cancer Centre currently offers non-medical prescribing (NMP) clinics for all solid tumour sites and there will be opportunities within a wide range of specialties offering opportunities for development



Work as an autonomous prescriber utilising clinical assessment, medication and diagnostic skills to provide and prescribe care and treatment for patient receiving anti-cancer therapies, within a pre-determined/agreed scope of practice



Support the Virtual Assessment Patient Service as an independent prescriber



Support education and development of other staff within scope of practice and ensure evidence-based use of medicines



Provide expert clinical advice and work collaboratively with other health professionals



Undertake service evaluation, improvement and audit



The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
